摘要
目的探讨经腹联合经阴道超声检查对异位妊娠的诊断价值。方法选择异位妊娠患者195例，根据随机数字表法将其分为A、B、C三组，每组65例。A组采用经腹超声检查，B组采用经阴道超声检查，C组采用经腹联合经阴道超声检查，比较三组异位妊娠检出率及异位妊娠声像图特征。结果A、B、C组异位妊娠检出率分别为75.38%、87.69%、96.92%，三组比较差异有统计学意义（P〈0.05）。C组检出宫外孕囊影中可见胎心及胎芽、宫外疑似孕囊影及宫外有混合性团块情况高于A组，差异有统计学意义（χ^2＝27.546、9.631、33.539，P〈0.05）；C组检出宫外孕囊影中可见胎心及胎芽、宫外疑似孕囊影情况与B组比较，差异未见统计学意义（χ^2＝0.284、0.000，P〉0.05）。结论经腹联合经阴道超声能有效提高异位妊娠检出率，可以更清晰地显示异位妊娠声像图特征，值得临床推广应用。
Objective To study the diagnostic value of transabdominal sonography （TAS） combined with transvaginal sonography （TVS） in ectopic pregnancy.Methods One hundred and ninety-five patients with ectopic pregnancy were divided into three groups by random number table method, with 65 cases in each group. Group A was examined by TAS, group B was examined by TVS, while group C was examined by TAS combined with TVS. The detection rate of ectopic pregnancy, ultrasonic characteristics of ectopic pregnancy were compared among the three groups.Results The detection rate of ectopic pregnancy was 75.38% in group A, 87.69% in group B and 96.92% in group C, the difference in detection rate among the three groups was statistically significant （P〈0.05）. The detection rates of visible fetal heart and buds in ectopic pregnancy cyst shadow, suspected ectopic pregnancy cyst shadow or mixed mass in group C were higher than those in group A （χ^2=27.546, 9.631, 33.539, P〈0.05）; there was no significant difference in detection rates of visible fetal heart and buds in ectopic pregnancy cyst shadow and suspected ectopic pregnancy cyst shadow between group B and group C （χ^2=0.284, 0.000, P〉0.05）.Conclusions TAS combined with TVS can effectively improve the detectione rate of ectopic pregnancy, and clearly show ultrasonic characteristics of ectopic pregnancy, which is worthy of clinical application and promotion.
