摘要
目的调研成都市基本公共卫生服务绩效考核实施现状,分析绩效考核指标完成情况存在的问题,为进一步完善绩效管理提出参考建议。方法采取整群随机抽样法,从成都市3个圈层的21个区县中,每个圈层抽取1个区(县)10家基层医疗卫生服务机构、共200名医务人员。通过实地问卷调查和深入访谈,收集上述3个调查地点2013-2015年的基本公共卫生服务数据进行分析,评估成都市基本公共卫生服务绩效考核指标完成情况,医务人员的收入情况、对绩效考核的认知情况以及患者满意度。结果实施基本公共卫生服务绩效考核以来,全市基本公共服务项目实施效果逐年提高,规范化电子健康档案建档率、儿童健康管理率、接种门诊达标率、老年人健康管理率、慢性病患者健康管理率和满意度逐年上升。2015年,94.5%(189/200)的医务人员认为实行绩效考核后工资收入升高,但满意度不高,72.3%(145/200)的基层医务人员对绩效考核的认知度不高。结论成都市实施基本公共卫生服务绩效考核促进了基本公共卫生服务项目的落实和较好的开展,但部分指标完成率相对较低。加大完成率较低基本公共卫生服务项目的组织实施力度,强化基本公共卫生服务绩效考核力度和导向性,应是下一步基本公共卫生服务绩效考核管理工作的重点。
Objective To investigate the implementation status of performance evaluation of basic public health service in Chengdu, and analyze the existing problems in performance appraisal, so as to put forward some suggestions for further improvement of performance management. Methods A cluster random sampling method was used, there were three layers in Chengdu with total 21 counties, in each layer there were 10 basic medical and health service institutes were selected from one county, and 200 medical staffs were selected from them. Through the questionnaire survey and in - depth interviews, collected and analyzed the service data from the upper mentioned 3 survey sites since 2013 to 2015 evaluated the performance of the basic public health service, the income of medical staff, the cognitive situation of performance assessment and the satisfaction of patients. Results since the implementation of basic public health services performance appraisal, the effect of basic public services was improved year by year, such as the rate of normalized electronic health records, children's health management rate, vaccination outpatient service rate, the elderly health management rate, management rate of patients with chronic diseases, and the satisfaction rate were increased year by year. In 2015, 94.5% (189/200) of medical staff believed that after the implementation of performance appraisal, wage income increased, but the satisfaction was not high, about 72.3% (145/200) of grass - roots medical staff awareness of performance appraisal were not high. Conclusion The implementation of performance appraisal of basic public health services in Chengdu promoted the practice and development of the basic public health service, but the completion rate of some indexes was relatively low. Strengthening the implementation on the low indexes projects of basic public health services, and strengthening the performance evaluation and guidance, should be the focus for the next step of the basic public health service performance appraisal management.
