摘要
目的:探讨短暂性脑缺血发作(TIA)患者ABCD2评分、颅内动脉狭窄程度与近期预后的相关性。方法:选取2015年6月-2017年6月在笔者所在医院就诊的TIA患者156例,根据影像学检查判断患者颅内动脉狭窄程度及ABCD2评分,并比较患者7 d内脑梗死发生率。结果:156例TIA患者中近7 d内发生脑梗死31例,占比为19.87%。TIA患者ABCD2评分等级越高,发生脑梗死占比越高,差异有统计学意义(P<0.05);TIA患者脑供血动脉狭窄程度越重,发生脑梗死占比越高,差异有统计学意义(P<0.05);狭窄≥50%且ABCD2评分≥4分脑梗死率发生和狭窄<50%且ABCD2评分<4分比较,差异有统计学意义(P<0.05)。结论:ABCD2评分和颅内动脉狭窄程度对TIA患者近期预后具有一定的指导意义,颅内动脉狭窄程度越严重,ABCD2评分越高,发生脑梗死的概率越大,要尽早采取预防措施。
Objective：To study the correlation between intracranial vessel stenosis degree and ABCD2 score in patients with transient ischemic attack（TIA） and short-term prognosis.Method：A total of 156 TIA patients from June 2015 to June 2017 in the author＇s hospital were selected,according to the examination of imaging,the degree of intracranial vascular stenosis was judged,ABCD2 score for patients were graded,the 7 d incidences of cerebral infarction in patients with different situations were compared.Result：31 cases of cerebral infarction occurred within 7 d in 139 patients with TIC,the proportion was 19.87%.The higher the grade of ABCD2 score in patients with TIA,the higher the proportion of patients with cerebral infarction,there was statistical significance between the ABCD2 score（P〈0.05）.The more severe cerebral artery stenosis in patients with TIA,the higher the proportion of patients with cerebral infarction,the degree of cerebral artery stenosis was statistically significant（P〈0.05）.The occurrence ratio of cerebral infarction in the situation of the degree of stenosis≥50% and the ABCD2 score≥4 points,compared with that in the situation of the degree of stenosis50% and the ABCD2 score4 points,the difference was statistically significant（P〈0.05）.Conclusion：The degree of intracranial vascular stenosis and ABCD2 score has certain guiding significance for the short-term prognosis of patients with TIA,he more stenosis the degree intracranial vascular,the higher the ABCD2 score,and the probability of occurrence of cerebral infarction is greater,precautionary measures should be carried out as soon as possible.
