Individualization of Phenytoin Dosage and Single-point Dosage Prediction

摘要 苯妥英钠可用一对剂量——稳态浓度数据来估算达到期望的治疗浓度所需剂量。经采用四种方法,即轨迹图法(Orbit plot method)、RD参数法(Richens and Dunlop nomog-ram)、Km=4参数法和群体清除率法(Population clearance method)。对10例服用苯妥英钠的癫痫病人进行剂量调整结果表明,其中轨迹图法最佳。 To dose the anticonvulsant phenytoin in clinical situation is difficult because ofthe non-lincar metabolism of the drug. In the rceent years, there are techniques tocalculate the dose needed to achieve a desired phenytoin concentration at steady-statebased on only one observed dose-steady-state concentration pair. To show whichmethod is more accurate in making routine phenytoin dosage adjustment, we comp-ared the four methods of these in 10 out-patients treated with phenytoin. They areOrbit plot method, Richens and Dulop nomogram, Km=4 nomogram and Populationclearance method. The result of statistical analysis shows that the Orbit plotmethod is the most accurate of the four compared methods.
