摘要
目的探究经阴道彩色多普勒超声(TVCDS)联合宫腔镜检查(HS)对绝经后妇女子宫出血的诊断效能。方法选取2014年11月-2017年3月我院疑似绝经后子宫出血患者124例,均行TVCDS、HS,并于HS后取子宫内膜组织,行病理组织学检查,将其作为诊断金标准。观察对比TVCDS、HS单独及联合检查特异度、敏感度、准确性、阳性及阴性预测值。结果 TVCDS、HS联合诊断敏感度96.15%(50/52)高于TVCDS诊断82.69%(43/52)、HS诊断84.62%(44/52),阴性预测值97.18%(69/71)高于TVCDS诊断87.67%(64/73),差异有统计学意义(P<0.05),TVCDS、HS单独及联合诊断特异度、准确性及阳性预测值相比,无明显差异(P>0.05)。结论对疑似绝经后子宫出血患者,联合采用TVCDS、HS诊断,可有效提高诊断敏感度,诊断效能较高,在临床诊断中具有重要意义。
Objective To explore the effect of transvaginal color Doppler ultrasonography（TVCDS）combined with hysteroscopy（HS）on the diagnostic efficacy in postmenopausal patients with uterine bleeding.Methods A total of124 cases of suspected postmenopausal uterine bleeding from November 2014 to March 2017 were selected and treated with TVCDS and HS,and endometrial tissues were taken after HS.Histopathological examination was used as the diagnostic standard.The specificity,sensitivity,accuracy,positive and negative predictive value of TVCDS and HS single and combined detection were observed and compared.Results The combined diagnostic sensitivity of TVCDS and HS96.15%（50/52）was higher than that of TVCDS diagnosis 82.69%（43/52）and HS diagnosis 84.62%（44/52）,and the negative predictive value 97.18%（69/71）was higher than that of the TVCDS diagnosis 87.67%（64/73）.The differences were statistically significant（P〈0.05）.There were insignificant differences in the diagnostic specificity,accuracy and positive predictive value between the diagnosis of TVCDS and HS alone and in combination（P〈0.05）.Conclusion TVCDS combined with HS examination on treating postmenopausal patients with uterine bleeding can effectively improve the diagnostic sensitivity.It has high diagnostic efficiency and it has an important significance in clinical diagnosis.
