摘要
目的观察电针结合物理疗法对恢复期脑卒中后足下垂患者平衡功能的影响。方法选取2016年1月至2017年1月期间在广东省佛山市中医院针灸科住院的76例恢复期脑卒中后足下垂患者,按"不平衡指数最小分配原则"分为观察组(电针+物理疗法)、对照组(物理疗法),每组38例。两组患者均进行基础治疗,并在治疗前及治疗4周后,分别采用关节活动度(ROM)、改良的Ashworth痉挛量表(MAS)、10米最大步行速度测定(MWS)、Berg平衡量表(BBS)对患者下肢痉挛程度、平衡功能进行评定。结果治疗后,两组患者足背屈ROM、MWS、BBS评分均较治疗前显著增加,MAS分级评分均较治疗前显著降低,差异均有统计学意义(P<0.05);治疗后组间比较,观察组足背屈ROM、MWS明显高于对照组(P<0.05),观察组MAS分级评分与对照组比较,差异无统计学意义(P>0.05),观察组BBS评分较对照组明显增加(P<0.05)。结论电针结合物理治疗可有效改善患者足背屈关节活动度,提高患者平衡功能。
Objective To observe the effect of electroacupuncture combined with physiotherapy on balance function ofconvalescent cerebral apoplexy patients with foot dropping. Methods 76 cases of convalescent cerebral apoplexy with footdropping treated in Department of Acupuncture and Moxibustion,Foshan Chinese Medicine Hospital of GuangdongProvince,from January 2016 to January 2017 were taken as subjects of the study. Patients were randomly divided intoobservation group(electroacupuncture combined with physiotherapy)and control group(physiotherapy)according to theprinciple of"minimum distribution of unbalanced index",38 cases in each one. Patients of both groups were given basictreatment. The range of motion(ROM),modified Ashworth spasticity scale(MAS),10 meters maximum walking speedmeasurement(MWS)and Berg balance scale(BBS)were used to assess the spasticity and balance function of the lowerextremities before and after 4 weeks treatment. Results The scores of dorsiflexion of the foot ROM,MWS and BBS in thetwo groups were significantly higher after treatment than those before the treatment,the score of MAS was significantly lowerthan that before treatment(P〈0.05);the scores of dorsiflexion of foot ROM and MWS in observation group were higher thanthe control group after treatment(P〈0.05);there was no statistically significant difference in the MAS grading score in theobservation group compared with the control group(P〈0.05). The score of BBS in the observation group was significantlyhigher than that of the control group(P〈0.05). Conclusion Electroacupuncture combined with physiotherapy caneffectively improve the range of motion of foot dorsiflexion and balance function.
