摘要
目的观察消肾清炎汤联合还原型谷胱甘肽(GSH)治疗小儿急性肾小球肾炎的疗效及对患儿血清血管内皮生长因子(VEGF)、巨噬细胞集落刺激因子(GM-CSF)和肿瘤坏死因子α(TNF-α)的影响。方法选择122例急性肾小球肾炎患儿,按照随机数字表法分为2组,每组各61例。2组均予利尿剂、抗生素等对症治疗,对照组加注射用GSH治疗;治疗组在对照组治疗基础上加消肾清炎汤治疗。2组均治疗7 d后统计临床疗效,并比较2组治疗前后肾功能[血尿素氮(BUN)、血肌酐(Cr)及24 h尿蛋白定量]及血清VEGF、GM-CSF、TNF-α水平变化。结果 2组治疗后BUN、Cr、24 h尿蛋白定量及血清VEGF、GM-CSF、TNF-α水平均较本组治疗前明显降低(P<0.05),且治疗组低于对照组(P<0.05)。治疗组总有效率90.16%,对照组75.41%,治疗组临床疗效优于对照组(P<0.05)。结论消肾清炎汤联合GSH能够有效降低急性肾小球肾炎患儿血清VEGF、GM-CSF和TNF-α水平,改善肾功能,提高临床疗效。
Objective To observe the curative effects of Xiaoshen Qingyan decoction combined with reduced glutathione hormone(GSH) on acute glomerulonephritis in children and its effect on serum macrophage colony stimulating factor(GM-CSF),vascular endothelial growth factor(VEGF) and tumor necrosis factor-α(TNF-α).Methods 122 cases of children with acute glomerulonephritis were divided into treatment group and control group according to random number table method,61 cases in each group.Both groups were treated by symptomatic treatment such as diuretics and antibiotics.The control group was treated by GSH,and the treatment group was treated by Xiaoshen Qingyan decoction on the basis of control group treatment,continuously treatment for 7 d.The TCM syndrome scores,renal function including blood urea nitrogen(BUN),serum creatinine(Scr),24 h urine protein quantitative and the changes of VEGF,GM-CSF and TNF-α before and after treatment were observed in two groups.Results The TCM symptom scores,and the levels of BUN,Scr,24 h urine protein quantitative,VEGF,GM-CSF and TNF-α after treatment in both groups were lower than those before treatment(P < 0.05),and which in the treatment group was lower than that in control group(P < 0.05).The total effective rate in treatment group(90.16%) was superior to that in control group(75.41%,P < 0.05).Conclusion Xiaoshen Qingyan decoction combined with GSH can effectively reduce the levels of serum VEGF,GM-CSF and TNF-α in children with acute glomerulonephritis,improve renal function and clinical efficacy.
