摘要
目的对局部应用氨甲环酸对减少全髋关节置换术后出血的有效性及安全性进行分析。方法 88例行全髋关节置换术患者,随机分为对照组与观察组,每组44例。观察组患者采用局部应用氨甲环酸进行止血处理,对照组采用常规的止血方式进行处理。比较两组的出血情况及各项指标。结果观察组患者引流瓶引流量、输血量以及总失血量均少于对照组,差异具有统计学意义(t=29.19、12.82、22.79,P<0.05)。观察组患者术后急查、术后24 h的红细胞计数及血红蛋白均明显高于对照组,差异具有统计学意义(P<0.05)。观察组术后24 h的纤维蛋白原、凝血酶原时间、活化部分凝血酶时间均优于对照组,差异具有统计学意义(P<0.05);观察组下肢静脉栓塞发生率为6.82%,与对照组的4.55%比较差异无统计学意义(P>0.05)。结论于行全髋关节置换术患者而言,局部应用氨甲环酸进行止血,不仅能有效减少患者术后出血量,还能降低了并发症发生率,从而进一步提高了药物的疗效。
Objective To analyze the effectiveness and safety of local application of tranexamic acid in reducing bleeding after total hip arthroplasty.Methods A total of 88 patients with total hip arthroplasty were randomly divided into control group and observation group,with 44 cases in each group.The observation group received local application of tranexamic acid,and the control group received conventional hemostasis method.The bleeding condition and indicators in two groups was compared.Results The observation group had less drainage volume by odrainage bottle,blood transfusion and total bleeding volume than the control group,and their difference was statistically significant(t=29.19,12.82,22.79,P〈0.05).The observation group had obviously higher postoperative emergency check,postoperative 24 h red blood cell count and hemoglobin than the control group,and their difference was statistically significant(P〈0.05).The observation group had better postoperative 24 h fibrinogen,prothrombin time and activated partial thrombin time than the control group,and their difference was statistically significant(P〈0.05).The observation group had incidence of lower extremity venous thrombosis as 6.82%,and it had no statistically significant difference comparing with 4.55% in the control group(P〈0.05).Conclusion For patients undergoing total hip arthroplasty,local application of tranexamic acid not only can effectively reduce postoperative bleeding volume,but also reduce the incidence of complications,thus further improving the efficacy of the drug.
