摘要
目的：探讨2型糖尿病并发脑梗塞康复期患者的中医情志护理体会。方法：选取2015年10月—2016年11月期间笔者所在社区58例2型糖尿病合并脑梗塞（CI）出院后康复期病人作为受试者,随机将58例患者分为两组：干预组（n=29）和对照组（n=29）。对照组接受内科规范化护理。护理组在规范化护理基础上按照中医辨证分型加强情志护理。护理后分别测量空腹血糖和餐后2 h血糖;心理健康运用抑郁自评量表（SDS）、焦虑自评量表（SAS）评估;生活能力状态运用SF-36健康量表。结果：两组2型糖尿病合并CI病人护理前血糖和血压各指标水平变化无明显差异,经中医情志护理后,干预组的血糖和血压各指标水平控制效果更佳,与护理前比较差异明显（r=5.384,P=0.005）,相较于对照组改善更显著（r=7.538,P=0.001）。两组2型糖尿病合并CI病人护理前SF-36评分变化无明显差异,经中医情志护理后,干预组的SF-36健康量表指标水平则显著上升,与护理前比较差异明显（r=6.428,P=0.003）,相较于对照组差异有显著性（r=6.910,P=0.001）。护理后干预组的SDS及SAS的指标水平均大幅度降低,相较于对照组差异有显著性（r=7.801,P=0.001）。护理后干预组再入院率为3.4%显著低于对照组27.5%（r=5.384,P=0.001）。结论：对2型糖尿病合并CI康复期的患者实施中医情志护理具有重要的临床意义,可良好的控制血糖和血压水平,改善负性心理状态,提升其生活能力,减少再入院率,值得推广应用。
Objective：To explore the emotional nursing of Chinese medicine in the rehabilitation stage of type 2 diabetes mellitus complicated with cerebral infarction.Methods：From October 2015 to November 2016,58 patients with T2DM combined with cerebral infarction（CI）rehabilitation were selected as subjects,and 58 patients were randomly divided into two groups：intervention group（n=29）and control group（n=29）.The control group was received standardized medical care.On the basis of standardized nursing,the nursing group should strengthen emotional nursing according to the syndrome differentiation of TCM.After nursing,fasting blood glucose and postprandial 2 h blood glucose were measured.Psychological health was assessed by the self rating depression scale（SDS）and self rating anxiety scale（SAS）,and the living capacity status was assessed by the SF-36 health scale.Results：There was no significant difference between the two groups in T2DM and CI patients before blood glucose and blood pressure level of each index,the TCM mental nursing intervention group,blood glucose and blood pressure level of each index control effect were better,clear difference compared with before treatment（r=5.384,P=0.005）,compared with the control group improved more significantly（r=7.538,P=0.001）.No significant difference of SF-36 score of nursing patients with T2DM complicated with CI of two groups before and after intervention by TCM mental nursing,SF-36 health scale index level group increased significantly,clear difference compared with before treatment（r=6.428,P=0.003）,there was significant difference compared with the control group（r=6.910,P=0.001）.The levels of SDS and SAS in the intervention group were significantly lower than those in the control group（r=7.801,P=0.001）.The re-admission rate of the nursing intervention group was 3.4%,significantly lower than that of the control group（27.5%,P=0.001,r=5.384）.Conclusion：TCM mental nursing of convalescent CI patients has important clinical significance for T2DM with good control of blood glucose and blood pressure levels,improve the negative mental state,improve their living ability,reduce the readmission rate.It is worth popularizing.
