摘要
背景:随着医患关系的日趋紧张,医患沟通在医疗过程中显得尤为重要,而术前谈话是外科手术科室医患沟通的最重要部分。目的:探索多媒体视频在骨科术前谈话中的临床应用,进一步研究在骨科术前谈话中潜在的医患纠纷问题以及防范对策,从而提高医患沟通水平,提高患者的满意度,降低医疗纠纷的发生率。方法:选取2015年9月至2016年9月收治的骨科手术患者600例,随机分成2组,分别采用不同的术前谈话方法。第1组(300例)为实验组,采用自制多媒体视频术前谈话;第2组(300例)为对照组,采用传统口述讲解术前谈话。从患者及家属对谈话的配合程度、提问情况,对谈话的满意度,对谈话内容了解程度,对手术的恐惧程度和术后主动进行功能训练时间共五方面进行评价。结果:术前采用自制多媒体视频进行谈话沟通较传统口述讲解的可操作性强,谈话内容标准化、专业化;可以使医师与患者及家属对于患者病情、手术流程、手术风险及预后的术前沟通更加充分,使患者对骨科手术有一个直观形象的了解,减轻对陌生手术的恐惧感,树立术后恢复功能的信心;使患者及其家属能够更加积极主动地配合治疗、护理和康复工作,加速疾病恢复。结论论:多媒体视频术前谈话比传统术前谈话更具优越性,可操作性强,值得临床推广和应用。
Background: The doctor-patient communication is becoming particularly important in the medical process with the increasing tension between doctors and patients. Preoperative conversation is the most important part of doctor-patient communication in surgical departments. Objective: To explore the application of multimedia video conversation before ortho- pedic surgery and potential doctor-patient disputes from the conversation as well as preventive measures, so as to improve the doctor-patient communication and patient's satisfaction and reduce the incidence of medical disputes. Methods: From September 2015 to September 2016, 600 patients admitted to the department of orthopedics surgery were randomly divided into 2 groups (n=300). Self-made multimedia video and doctor oral explanation were performed for preoperative conversa- tion in the experimental group and control group, respectively. The atmosphere of the conversation, questioning, satisfaction, fear of surgery, and postoperative active function training time were assessed. Results: Preoperative doctor-patient communi- cation using multimedia video was more operable, standardized and specialized as compared with the traditional oral inter- pretation. The status of the disease, surgical procedures, operative risk and prognosis could be fully explained by multimedia videos. The visual image could reduce the fear for operation, establish the confidence of postoperative recovery function, make patients and their families actively cooperate with the treatment, nursing and rehabilitation, and accelerate the recov- ery. Conclusions: Multimedia video doctor-patient communication is superior to traditional preoperative oral explanation.
