摘要
目的对比不同剂量芬太尼对高血压患者行无痛支气管镜时麻醉效果的影响。方法选取2016年10月~2017年7月间于本院接受无痛支气管镜检查的125例高血压患者作为此次研究对象,依据两组患者接受麻醉药物的剂量不同分为A1组与A2组,A1组给予芬太尼2μg/kg,A2组给予芬太尼4μg/kg。观察两组患者不同时间段内血压、心率以及血氧饱和度的变化、术中术后不良反应发生率。结果A1组T_1、T_2、T_3、T_4时段的HR、SBP、DBP以及SpO_2与T_0相比差异有统计学意义,而A2组的HR与SpO_2在T_2、T_3时段内有一定差异,SBP、DBP水平相比A1组更接近正常,波动幅度明显小于A1组,差异有统计学意义意义(P<0.05);且A2组术中呛咳体动及术后并发症发生率合计明显低于A2组,差异有统计学意义意义(P<0.05)。结论芬太尼作为临床常用的麻醉药物,镇痛效果强,无蓄积且安全可靠,为临床麻醉高质量、高可控性提供了选择,在高血压患者行无痛支气管镜麻醉时,4μg/kg芬太尼是比较合适的剂量。
Objective To compare the clinical anesthesia effect of different doses of Fentanyl in the hypertension patients during the painless bronchoscopy. Methods 125 hypertension patients of receiving the painless bronchoscopy from 2016.10 to 2017.7 in our hospital were selected. The subjects were divided into Group A1 and Group A2 according to the different doses of Fentanyl. Group A1 took 2 μg/kg of Fentanyl; Group A2 took4 μg/kg of Fentanyl. At the different time periods, the blood pressure, HR and SpO_2 as well as the adverse event rate for two groups was compared.Results At T_1, T_2, T_3 and T_4, the index of HR, SBP, DBP and SpO_2 for Group A1 was significantly different from T_0; at T_2 and T_3, the index of HR and SpO_2 for Group A2 was different; at T_2 and T_3, the index of SBP and DBP for Group A2 was better than Group A1. The related index for Group A2 was significantly more stable than Group A1(P<0.05); during the surgery, the incidence rate of bucking and postoperative complications for Group A2 was significantly lower than Group A1(P<0.05). Conclusion As a commonly used anesthesia drug, Fentanyl is featured as high effect,no accumulation, high safety and reliability and controllability. During the painless bronchoscopy, 4μg/kg of Fentanyl is recommended.
