摘要
目的:探讨综合护理干预对手足口病(HFMD)患儿Fas-Fas L表达水平的影响。方法:选择我院儿科2015年3月~2016年8月肠道病毒71型(EV71)感染合并神经系统损伤的重症手足口病患儿37例,在综合治疗的基础上给予综合护理干预,记录患儿退热时间、症状体征缓解时间、住院时间及并发症发生情况,治疗3,7 d时检查患儿外周血淋巴细胞Fas-Fas L表达。结果:37例患儿无1例死亡,平均退热时间(25.7±4.8)h,症状体征缓解平均时间(3.8±1.5)d,平均住院时间(9.2±1.3)d,无1例发生感染等并发症。治疗3,7 d患儿Fas-Fas L水平低于治疗前,治疗7 d低于治疗3 d,差异均有统计学意义(P<0.05)。结论:在积极治疗的同时配合综合护理干预可迅速降低患儿Fas-Fas L水平,减轻神经系统损害,对促进病情康复、提高疗效有积极的作用。
Objective: To explore the influence of comprehensive nursing intervention on Fas-Fas L expression level for child patients with hand-foot-mouth disease( HFMD). Methods: To select 37 cases of child patients with hand-foot-mouth disease in serious condition caused by human enterovirus71( EV71) infection associated with neurological injury cured in pediatric department of our hospital from March 2015 to August 2016. Apply comprehensive nursing intervention based on comprehensive treatment,record the time of abatement of fever,response duration of symptoms and signs,hospital stays and occurrence conditions of complications,and examine patients' peripheral blood lymphocyte Fas-Fas L on the third and seventh day after treatment. Results: No one died among 37 cases,average time of abatement of fever was( 25. 7 ± 4. 8) h,average response duration of symptoms and signs was( 3. 8 ± 1. 5)d,average hospital stays was( 9. 2 ± 1. 3) d and no infection and complications occurred. Level of patients' Fas-Fas L on the third and seventh day after treatment was lower than before,and level of patients' Fas-Fas L on the seventh day was lower than that on the third day,and the difference was statistically significant( P 〈0. 05). Conclusion: Active treatment combined with comprehensive nursing intervention could quickly reduce child patients' Fas-Fas L level,reduce damage to nervous system,and play a positive effect on promoting state of illness and curative effect.
