摘要
目的探讨房水中血管内皮生长因子(VEGF)、转化生长因子β2(TGFβ_2)、单核细胞趋化因子1(MCP-1)、白细胞介素-6(IL-6)、IL-8、IL-1β与白内障合并2型糖尿病(T2DM)患者术后并发黄斑水肿(ME)的关系。方法选取我院手术治疗的白内障合并T2DM患者180例(180眼)(时间2014年1月至2016年1月)进行回顾性分析,其中38例(38眼)术后并发ME(ME组)、142例(142眼)术后未并发ME(非ME组),检测两组术后房水中VEGF等指标并进行比较分析。结果 ME组房水中VEGF、TGFβ_2、MCP-1、IL-6、IL-8、IL-1β水平均显著的高于非ME组,差异具有统计学意义(P<0.05);ME 1级组患者房水中VEGF、TGFβ_2、MCP-1、IL-6、IL-8、IL-1β水平均显著的低于ME 2级组,差异具有统计学意义(P<0.05);ME患者的黄斑中心凹厚度与房水中VEGF、TGFβ_2、MCP-1、IL-6、IL-8、IL-1β水平均呈显著的正相关关系(P<0.05)。结论白内障合并T2DM患者术后并发ME与房水中VEGF及相关炎症因子水平升高关系密切。
Objective To investigate the relevance between VEGF,TGF beta 2,MCP-1,IL-6,IL-8,IL-1 beta and macular edema(ME) in patients simultaneously with cataract and type 2 diabetes mellitus(T2 DM) after aqueous surgery. Methods 180 patients(180 eyes) with cataract and T2DM from January 2014 to January 2016 in our hospital were retrospectively analyzed,including 38 cases(38 eyes) with postoperative complication of ME(ME group),142 cases(142 eyes) without that(non ME group). The level of cytokines in aqueous humor in the two groups were detected and compared. Results The levels of VEGF,TGF,MCP-1,IL-6 beta 2,IL-8,IL-1 in aqueous humor were significantly higher in ME group than those in non ME group(P <0.05);in ME grade 1 group,the levels of VEGF,TGF beta 2,MCP-1,IL-6,IL-8,IL-1 aqueous humor were significantly lower than those in ME grade 2 group(P<0.05);the thickness of macular foveal in ME were positively correlated with the levels of VEGF,TGF,MCP-1,IL-6 beta 2,IL-8,IL-1 in aqueous humor(P<0.05). Conclusion the ME after cataract surgery in patient with cataract and T2 DM is associated with elevated levels of VEGF and inflammatory factors in aqueous humor.
