摘要
目的探讨单核细胞趋化蛋白-1(MCP-1)、可溶性白细胞介素-2受体(s IL-2R)和干扰素-γ(IFN-γ)含量在儿童急性白血病不同分型中的变化规律及联合检测的诊断意义。方法选取2014年1月至2017年5月恩施土家族苗族自治州中心医院收治的儿童急性白血病患者80例为观察组,其中急性淋巴细胞白血病(ALL组)40例,急性髓细胞白血病40例(AML组),同期体检健康者40例为对照组,检测并比较各组受检者血清中MCP-1、s IL-2R和IFN-γ的含量。结果 ALL组和AML组患者血清中MCP-1和s IL-2R含量分别为MCP-1(18.43±1.12)ug/L、(694.54±17.52)U/m L,(15.62±1.07)μg/L、s IL-2R(525.78±14.59)U/m L,明显高于对照组的(6.12±0.55)μg/L、(175.48±12.46)U/m L,差异均具有统计学意义(P<0.05);AML组患者血清中MCP-1和s IL-2R含量明显低于ALL组,差异均有统计学意义(P<0.05);ALL组和AML组患者血清中IFN-γ含量分别为(3.72±0.85)pg/m L和(8.37±0.76)pg/m L,明显低于对照组的(11.19±0.84)pg/m L,差异均具有统计学意义(P<0.05);AML组患者血清中IFN-γ含量明显高于ALL组,差异具有统计学意义(P<0.05);ALL组和AML组患者血清中MCP-1、s IL-2R和IFN-γ的联合检出率、灵敏度和特异性均明显高于各指标单独检出率,差异均具有统计学意义(P<0.05)。。结论儿童急性白血病患者血液中MCP-1、s IL-2R和IFN-γ呈异常表达状态;且MCP-1、s IL-2R和IFN-γ联合检测在儿童急性白血病不同分型的诊断中具有重要临床意义。
Objective To investigate the change of monocyte chemoattractant protein-1 (MCP-1), soluble in-terleukin-2 receptor (sIL-2R) and interferon-γ (IFN-γ) in children with acute leukemia (AL) of different types and thediagnostic significance of the combined detection. Methods Eighty children with AL who were admitted in Enshi Tujiaand Miao Autonomous Prefecture Central Hospital from January 2014 to May 2017 were selected as observation group,including 40 children of acute lymphoblastic leukemia (ALL group), and 40 children of acute myeloid leukemia (AMLgroup). Forty healthy children were taken as control group at the same period. The contents of MCP-1, sIL-2R andIFN-γ were detected and compared between the three groups. Results The levels of MCP-1 and sIL-2R were (6.12±0.55) μg/L and (175.48 ± 12.46) U/mL in the control group, which were significantly lower than (18.43 ± 1.12) μg/L and(694.54±17.52) U/mL in ALL group and (15.62±1.07) μg/L and (525.78±14.59) U/mL in AML group (P〈0.05). Comparedwith ALL group, serum levels of MCP-1 and sIL-2R in AML group were significantly decreased (P〈0.05). The IFN-γ levelwas (11.19±0.84) pg/mL in control group, versus (3.72±0.85) pg/mL in ALL group and (8.37±0.76) pg/mL in AML group (P〈0.05). Compared with ALL group, serum levels of IFN-γ in AML group were significantly increased (P〈0.05). The com-bined detection rate, sensitivity and specificity of MCP-1, sIL-2R and IFN-γ ALL group and AML group were significantlyhigher than those of single detection of each index (P〈0.05). Conclusion MCP-1, sIL-2R and IFN-γ in children withacute leukemia were abnormally expressed. Combined detection of MCP-1, sIL-2R and IFN-γ has important clinical sig-nificance in the diagnosis of children with acute leukemia of different types.
