摘要
目的探讨超声弹性成像联合血清标志物在慢性乙肝肝纤维化评估中的价值。方法选取我院2015年7月~2017年5月于传染科收治的慢性乙肝患者87例,采用超声弹性成像联合血清标志物对患者进行检查,选取同期来医院体检的69例健康者作为对照组,采用血清标志物对患者进行检查。比较两组血清标志物检查结果,并评估慢性乙肝患者肝纤维化的分期情况。结果观察组透明质酸(HA)、层粘连蛋白(LN)和Ⅳ型胶原蛋白(IV-C)等血清学指标明显高于对照组,差异有统计学意义(P<0.05);患者肝纤维化程度与超声检测病理得分间存在正向相关性,差异具有统计学意义(r=0.864,P<0.05);联合检测与病理活检的结果对照,准确度为96.55%(84/87)。结论采用超声弹性成像联合血清标志物对慢性乙肝患者进行肝纤维化评估,效果十分显著;可有助于了解患者肝纤维化的发展进程,直观、准确地获知肝纤维化的严重程度,对患者治疗方案的选择和预后的指导有十分重要的价值。
Objective To explore the value of ultrasound elastic imaging in the evaluation of chrortic hepatitis b liver fibrosis. Methods A total of 87 cases of chronic hepatitis b patients admitted by our hospital fro~, July 2015 t~ May 2017 in the department of infectious diseases were selected, and the patients were examined by ultrasonic elastic imaging combined with serum markers, there were 69 healthy patients who were selected for the same period, the patients were examined by serum markers. The results of the two groups of serum markers were compared and the staging of hepatic fibrosis in chronic hepatitis b patients was evgluated. Results Observation group of hyaluronic acid （HA）, laminin（LN） and type IV collagen（IV-C） serological indexes such as significantly higher than the control group（P〈0.05）, the difference was statistically significant; There was a positive correlation between the degree of hepatic fibrosis and the pathological score of uhrasonography（r=0.864,P〈0.05）, and the difference was statistically significant; The results of the joint test were 96.55%（84/87） compared with the results of pathological biopsy. Conclusion The liver fibrosis assessment of chronic hepatitis b patients was evaluated by ultrasonic-elastic imaging combined with serum markers. Can help to understand the development of liver fibrosis in patients with process, intuitive and accurately know the severity of liver fibrosis, on the choice of treatment and prognosis of patients with guidance has very impgrtant value.
