摘要
目的探讨超声下导丝定位活检与真空辅助旋切活检在不可触及乳腺病灶诊断中的应用价值。方法将2015年1月~2016年12月在我院行超声检查的74例乳腺病患者按照超声下检查方法的不同分成观察组37例与对照组37例,观察组患者给予超声下导丝定位活检进行诊断,对照组患者给予超声下真空辅助旋切活检进行诊断,分别对两种超声下不同方法的诊断结果、手术时间、术后美容评估及并发症情况进行统计对比。结果超声下导丝定位活检与真空辅助旋切活检在纤维腺瘤、腺病、乳头状瘤、不典型增生、原位癌及浸润性癌中的检出结果差异不明显;观察组患者的手术时间明显长于对照组患者;观察组患者的术后乳房美容优良率明显低于对照组患者;观察组患者的术后并发症发生率明显低于对照组患者。结论超声下导丝定位活检与真空辅助旋切活检在不可触及乳腺病灶诊断中均具有较高的准确性,导丝定位活检操作简便、经济实用,临床适用性较高,但术后乳房美容效果一般。真空辅助旋切活检术后乳房美容效果较好,但费用较高难以普遍推广,且不适用于高危乳腺病灶患者。
Objective To investigate the value of ultrasound-guided wire positioning biopsy and vacuum assisted breast biopsy in the diagnosis of non-palpable breast lesions. Methods A total of 74 cases of breast diseases undergoing ultra sound in our hospital from January 2015 to December 2016 were divided into observation group（n=37） and control group （n=37）, according to the method of ultrasonic examination. The patients in observation group were given ultrasound- guided wire positioning biopsy. The patients in the control group were diagnosed by vacuum assisted breast biopsy un- der ultrasound. The diagnostic results, operation time, postoperative cosmetic evaluation and complications between the two methods were compared statistically. Results There was no significant difference in the detection results of fi- broadenoma, adcnosis, papilloma, atypical hyperplasia, carcinoma in situ and invasive carcinoma between ultrasound- guided wire positioning biopsy and vacuum assisted breast biopsy. The operative time in the observation group was sig- nificantly longer than that of the control group. The excellent rate of postoperative breast cosmetic effect in the observa- tion group was significantly lower than that of the control group. The incidence of postoperative complications in the observation group was significantly lower than that in the control group. Conclusion Ultrasound guided wire positioning biopsy and vacuum assisted breast biopsy have high accuracy in the diagnosis of non-palpable breast lesions. The guide wire positioning biopsy is simple, economical and practical, and its clinical applicability is high. However, its postoperative breast cosmetic effect is ordinary. The breast cosmetic effect after vacuum-assisted breast biopsy is better, but the cost is so high that it is difficult to universally promote and it is not suitable for high-risk patients with breast lesions.
