脂肪酸异常代谢在糖尿病心脏重构中的作用 被引量:10

摘要 糖尿病心脏重构是糖尿病的一个重要并发症之一,在心脏重构的早期,心脏会表现为左心室舒张功能异常,后期则出现心肌收缩功能下降,严重威胁患者生命安全。近年来糖尿病发病率逐渐增高,糖尿病心脏重构也越发多见。脂肪酸代谢异常是糖尿病心脏重构的一个重要致病因素。糖尿病时,机体会出现胰岛素抵抗,心肌细胞利用葡萄糖提供能量的过程受阻,转而趋向于全部依赖脂肪酸代谢生成ATP,这一能量代谢变化可致心肌功能障碍,使心肌细胞及某些细胞凋亡、非细胞成分(如胶原纤维)异常增加,导致心脏重构。 Diabetic myocardial remodeling is one of the important complications of diabetes. It causes left ventricular diastolic dysfunction in early time,and myocardial contractility decline later,which will threat the patients' life safety seriously. With the incidence of diabetes gradually increasing,diabetic myocardial remodeling is more common now. Abnormal fatty acid metabolism is an important pathogenic factor for diabetic myocardial remodeling. Under diabetes,insulin resistance occurs in the body,which can block the process of cardiomyocytes using glucose to provide energy,and cardiomyocytes tend to entirely rely on fatty acid metabolism to generate ATP. This energy metabolism change can cause myocardial dysfunction,and apoptosis of cardiomyocytes and some other cells,abnormal increase of non-cellular components( such as collagen fibers),which leads to cardiac remodeling.
