氟比洛芬酯用于宫颈癌手术中对细胞功能的影响

摘要 目的研究氟比洛芬酯用于宫颈癌手术中对细胞功能的影响。方法将本院2014年3月至2016年3月收治的择期行宫颈癌手术的86例患者按照随机数表法分为观察组和对照组,每组各43例。麻醉前10分钟,观察组患者静脉注射氟比洛芬酯,对照组患者静脉注射等体积生理盐水。两组患者采用相同的麻醉诱导方案、麻醉维持方案及术后镇痛方式。比较麻醉前30分钟(T0)、手术开始后2小时(T1)、术后第1天(T2)及术后第3天(T3)两组患者免疫细胞和炎性细胞因子水平变化情况及术后3、6、12、24小时的视觉模拟评分法(visual analogue scales,VAS)评分。结果 T1、T2、T3时刻,两组患者CD3^+、CD4^+、自然杀伤细胞(natural killer cell,NK细胞)水平及CD4^+/CD8^+均明显低于本组T0时刻水平(P<0.01),白介素-6(interleukin-6,IL-6)水平均明显高于本组T0时刻水平(P<0.01)。T1、T2、T3时刻,观察组患者CD3^+、CD4^+、NK细胞水平及CD4^+/CD8^+均明显高于对照组(P<0.01),IL-6水平均明显低于对照组(P<0.01)。T2、T3时刻,两组患者肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)水平均明显高于本组T0时刻水平(P<0.01),观察组患者TNF-α水平均明显低于对照组(P<0.01)。术后3小时,观察组患者VAS评分显著低于对照组(P<0.05)。结论麻醉前静脉注射氟比洛芬酯可以减轻创伤引起的细胞免疫抑制效应和术后早期疼痛,减少创伤引起的炎性细胞因子的生成。 ObjectiveTo explore the effects of Flurbiprofen axetil on cellular function for the cervical cancer surgery. Method86 patients who scheduled for cervical cancer surgery from March 2014 to March 2016 in our hospital were selected and divided into observation group and control group according to the random number table method, 43 cases in each group. 10 minutes before anesthesia, patients in observation group were given Flurbiprofen axetil with intravenous injection, patients in control group were given isometric physiological saline with intravenous injection. Both groups were treated with the same anesthesia inducton, maintenance of anesthesia and postoperative analgesia. The levels of immunocyte and infiammatory cytokines at 30 minutes before anesthesia (T0), 2 hours after operation began (T1), 1 day postoperatively (T2) and 3 days postoperatively (T3) were compared between the two groups. At 3 hours, 6 hours, 12 hours and 24 hours after operation, the visual analogue scales (VAS) score were compared between the two groups. ResultAt T1, T2 and T3, the levels of CD3+, CD4+, natural killer cell (NK cell) and CD4+/CD8+ were lower than those at T0 in the same group (P 〈 0.01), the levels of interleukin-6 (IL-6) were higher than those at T0 in the same group (P〈 0.01). At T1, T2 and T3, the levels of CD3+, CD4+, NK cell and CD4+/CD8+ in observation group were lower than those in control group (P 〈 0.01), the levels of IL-6 were higher than those of control group (P〈0.01). At T2 and T3, the levels of tumor necrosis factor-α (TNF-α) were lower than those at T0 in the same group (P 〈 0.01), the levels of TNF-α in observation group were lower than those in control group (P 〈 0.01). 3 hours after operation, the VAS score of observation group was significantly lower than that of control group (P 〈 0.05). ConclusionBefore anesthesia, using Flurbiprofen axetil with intravenous injection can reduce the cellular immunity inhibition efiect and cytokine production caused by surgical trauma and improve the early postoperative pain.
