320排CT灌注成像联合MVD在原发性肝癌诊断中的应用价值 被引量:4

An investigation on the clinical application of 320-slice spiral CT perfusion imaging combined with microvessel density (MVD) for the diagnosis of primary liver cancer

摘要 目的利用320排CT灌注成像(CTPI)探讨原发性肝癌的血流动力学特点及其与实体肿瘤内微血管密度(MVD)相关性。方法对43例患者进行CT灌注检查,38例原发性肝癌为观察组,5例正常肝脏为对照组,对两组CT灌注参数血流量(BF)、血容量(BV)、肝动脉灌注量(HAP)、肝动脉灌注指数(HPI)、门静脉灌注量(HPP)、平均通过时间(MTT)及毛细血管表面通透性(PS)进行血流动力学分析;取38例肝癌患者的手术标本和对应的癌旁组织进行免疫组织化学检测,并对微血管密度(MVD)进行计数。分析CT灌注参数与MVD计数的相关性。结果与对照组比较,观察组BF、BV、HAP、HPI、PS均明显升高,差异具有统计学意义(P<0.05);HPP、MTT均明显降低,差异具有统计学意义(P<0.05);肝癌的MVD计数均高于癌旁组织,差异具有统计学意义(P<0.05);通过Pearson相关分析显示,HAP与MVD呈正相关(r=0.556,P<0.05);HPI与MVD呈正相关(r=0.871,P<0.01),HPP与MVD间没有相关性(r=0.016,P>0.05)。结论肝脏CT灌注成像能有效评价原发性肝癌的血流动力学状态;HAP、HPI与MVD有正相关性,HPP与MVD间没有相关性。 Objective To investigate the hemodynamic characteristics of primary hepatocellular carcinoma (HCC)and its correlation with microvessel density(MVD)measured using 320-slice Spiral CT perfusion imaging (CTPI) . Methods A total of 43 subjects were enrolled in the study including 38 primary liver cancer patients in the observation group and 5 healthy volunteers in the control group. The CT perfusion parameters such as blood flow(BF), blood volume(BV), hepatic artery perfusion(HAP), hepatic artery perfusion index(HPI), portal vein perfusion(HPP), mean transit time(MTT)and capillary surface permeability(PS)were compared between two groups.The specimens and corresponding paracancerous tissues were examined by immunohistochemistry and MVD was counted. The correlation between CT perfusion parameters and MVD counts was analyzed. Results Compared with the control group, the levels of BF, BV, HAP, HPI and PS in observation group were significantly higher, the difference was statistically significant(P 〈 0.05); The levels of HPP, MTT in observation group were significantly lower than the control group, the difference was statistically significant(P 〈 0.05). The MVD counts in observation group were significantly higher than those in paracancer tissues(P 〈 0.05). Pearson correlation analysis showed that there was a significant positive correlation between HAP and MVD (r = 0.556, P 〈 0.05) . There was a significant positive correlation between HPI and MVD (r = 0.871, P 〈 0.01). There was no significant correlation between HPP and MVD(r = 0.016, P 〉 0.05). Conclusion Hepatic CT perfusion imaging can effectively evaluate the hemodynamic status of primary liver cancer. HAP, HPI and MVD have a positive correlation, but there is no significant correlation between HPP and MVD.
