摘要
目的:观察益气活血利水方治疗冠状动脉粥样硬化性心脏病慢性心力衰竭心肾阳虚证的临床疗效。方法:将126例冠状动脉粥样硬化性心脏病慢性心力衰竭心肾阳虚证患者随机数表法分为对照组和治疗组,每组63例。对照组采用常规西药治疗,治疗组在对照组治疗基础上加用益气活血利水方(黄芪30 g,红参7.5 g,川芎10 g,当归15 g,丹参20 g,桃仁10 g,白术15 g,红花10 g,茯苓15 g,桂心10 g,远志10 g,五味子15 g,甘草10 g,陈皮15g)治疗。比较两组患者治疗前后脑钠肽(brain natriuretic peptide,BNP)、左室射血分数(left ventricular ejection fractions,LVEF)、心力衰竭程度、临床症状、生存质量的变化情况,并评价两组患者的临床疗效。结果:治疗组有效率为92.07%,高于对照组77.78%,差异有统计学意义(P<0.05);与治疗前比较,两组患者BNP、Lee氏评分、中医证候积分、生存质量评分在治疗后均下降,而LVEF均上升,且治疗后治疗组上述观察指标均优于对照组,差异均有统计学意义(P<0.05)。结论:对冠状动脉粥样硬化性心脏病慢性心力衰竭心肾阳虚证患者采用益气活血利水方治疗,可显著提高临床疗效,缓解临床症状,改善心功能,提高生存质量。
Objective： To observe the clinical efficacy of Yiqi Huoxue Lishui prescription in treatment of heart-kidney Yang deficiency type of coronary heart disease（ CHD） with chronic heart failure（ CHF）. Methods： 126 patients with heart-kidney Yang deficiency type of CHD with CHF were divided into two groups by random number table,63 cases in each group. The control group took conventional western medicine,and the treatment group took Yiqi Huoxue Lishui prescription on the basis of control group.The prescription： Huangqi（ Astragalus） 30 g,Hongshen（ Red Ginseng） 7. 5 g,Chuanxiong（ Ligusticum Wallichii） 10 g,Danggui（ Chinese Angelica） 15 g,Danshen（ Salvia Miltiorrhizae） 20 g,Taoren（ Peach Kernel） 10 g,Baizhu（ Rhizoma Atractylodis Macrocephalae） 15 g,Honghua（ Carthamus Tinctorius） 10 g,Fuling（ Poria Cocos） 15 g,Guixin（ Ramulus Cinnamomi） 10 g,Yuanzhi（ Polygala Tenuifolia） 10 g,Wuweizi（ Schisandra Chinensis） 15 g,Gancao（ Licorice） 10 g,Chenpi（ Dried Orange Peel） 15 g. The changes of brain natriuretic peptide（ BNP）,left ventricular ejection fractions（ LVEF）,heart failure degree,clinical symptom,survival quality of two groups before and after treatment were compared,and clinical efficacy of two groups were evaluated. Results： The effective rate of treatment group was 92. 07%,which was higher than 77. 78% of control group（ P〈0. 05）; Compared with pretreatment,the BNP,Lee score,TCM symptom score,survival quality score of two groups decreased after treatment,but LVEF increased,and the above indexes of treatment group were better than control group（ P〈0. 05）. Conclusion： Yiqi Huoxue Lishui prescription for treating heart-kidney Yang deficiency type of CHD with CHF can significantly increase clinical efficacy,relieve clinical symptoms,improve cardiac function,promote body recovery and increase survival quality.
