摘要
目的探讨PTED治疗腰椎间盘突出症的临床疗效及对患者疼痛和免疫功能的影响。方法选择行手术治疗的126例腰椎间盘突出症患者为对象。根据患者治疗方法的不同,将126例患者分为观察组和对照组,各63例。对照组采用显微内镜下椎间盘切除术(MED)治疗。观察组采用经皮椎间孔镜下椎间盘切除术(PTED)治疗。观察患者的临床疗效及疼痛评分和免疫功能的变化情况。结果观察组手术切口长度、术中出血量及住院时间均显著低于对照组,差异具有统计学意义(P<0.05)。两组患者手术时间比较差异无统计学意义。两组患者术后VAS评分显著低于术前,比较结果有统计学意义(P<0.05)。两组患者术前术后VAS评分及MacNab疗效评价组间比较差异无统计学意义。两组患者治疗后血清IgG、IgA、IgM水平均较治疗前有明细下降,比较结果有统计学意义(P<0.05)。两组患者治疗前及治疗后血清IgG、IgA、IgM水平组间比较差异无统计学意义。结论 PTED术能减轻腰椎间盘突出症患者疼痛感、改善患者免疫功能,且具有手术创口小、术中出血少以及术后恢复快等特点,临床疗效满意。
Objective To investigate the clinical efficacy of PTED on lumbar disc herniation and its effects on pain and immune function of patients.Methods 126 patients with lumbar disc herniation were enrolled in this study.According to the different treatment of patients,126 patients were divided into observation group and control group(n=63).Patients in control group were treated by microendoscopic discectomy(MED).Patients in observation group were treated by percutaneous intervertebral discoscopy(PTED).Clinical curative effect and pain score and immune function of patients were observed.Results The length of incision,intraoperative blood loss and hospitalization time in the observation group were significantly lower than control group(P<0.05).There was no significant difference in the operation time between the two groups.The VAS score of patients in two groups were significantly lower after operation(P<0.05).There was no significant difference between the two groups in the VAS score and the evaluation of MacNab efficacy.Serum IgG,IgA and IgM levels in the two groups were significantly lower after treatment(P<0.05).There was no significant difference in serum IgG,IgA and IgM between the two groups before and after treatment.Conclusion PTED can reduce the pain of patients with lumbar disc herniation,and improve their immune function.It has the characteristics of small surgical wounds,less intraoperative blood loss and faster postoperative recovery.Its clinical curative effect is satisfactory.
