摘要
目的探讨胚胎发育不良性神经上皮瘤的MRI特征,为临床术前提供正确的诊断。方法回顾性分析11例DNET患者的临床、MRI和病理资料,对病例MRI特征进行分析。结果 11例DNET患者中位于额叶3例、顶叶3例,颞叶5例。所有患者均为单发于幕上大脑皮层,以皮层为基底,10例患者呈三角形,1例略呈椭圆形。肿瘤以囊性成分为主,1例患者内见条状出血,水抑制序列上病灶可见完整或不完整高信号环,DWI表现为低信号,ADC值明显增高。增强后6例为分隔强化,1例为结节强化。所有肿瘤均未见水肿带。3例病灶内Cho峰和Cr峰无改变,NAA峰较正常区域未见减低。结论 DNET的MRI表现具有一定的特异性,可以为临床术前提供一定的参考价值。
] Objective To explore imaging of features and diagnostic value of dysembryoplastic neuroepithelial tumor （DNET） with MRI. Methods Combining with clinical histopathology, MR imaging and histopathology, MR imaging of 11 DNET cases was retrospectively analyzed. Results All the DNETs were solitary （three frontal, three occipital and five parietal）. All patients were found in the supratentorial cerebral cortex, with cortex as the base, and 10 cases were triangular, 1 case was slightly oval. Tumors were mainly cystic components, 1 patient had striped hemorrhage, complete or incomplete high signal loops were seen on the water suppression sequence, DWI showed low signal and ADC value increased signifcantly. After enhancement, 6 cases were divided enhancement and 1 was nodular enhancement, and no edema was found in all tumors. In 3 cases, the Cho peak and Cr peak were not changed, and the NAA peak was not lower than that in the normal area. Conclusion DNET was characteristic on MRI, MRI imaging is important for preoperative planning.
