CT相对密度值评估胸腰椎骨折椎体内植骨密度与骨愈合的关系 被引量:3

The Relative Density of CT in Assessing the Relationship between Bone Graft Density and Bone Healing in Thoracic and Lumbar Vertebrae Fractures

摘要 针对CT测量相对密度值进而对腰椎骨折区域密度变化情况进行详细评估,进一步对椎体内植骨效果进行统计分析,最终为临床研究分析提供可行的方案。选取胸腰椎骨折32例患者,对椎体骨折部位进行撑开复位并植骨成固定修复。固定后对植骨区域进行CT检查,观察固定后1周,6个月的椎前后植骨区域的CT相对密度值、伤椎前后缘压缩率、伤椎Cobb角变化。研究表明,高、低密度胸腰椎骨折患者,固定后一个月伤椎前后缘压缩率和Cobb值与术前相比大幅度下降,且具有统计学意义,但是较6个月后的最终回访值略高,但无统计学意义,说明患者在术后病症恢复,但在恢复期无明显改变,也未产生二次伤害。此外,这两组的伤椎前后缘压缩率和Cobb值变化基本一致,说明疗效一致。高、低密度组患者最后随访的植骨区域的CT相对密度值明显高于术后一周的,说明植骨区域密度逐渐增大,成骨量逐渐增加。低密度组骨折植骨区域密度在术后一周以及最后随访时均高于高密度组,但是最后随访时的高低密度组的CT相对密度值无显著性差异,其值较为接近。因此,可根据患者手术前后的CT相对密度值来评估其骨折愈合情况,为胸腰椎骨折患者提供临床指导依据。 To measure the relative density with CT, the density change of lumbar fracture area was evaluated in detail, and the bone grafting effect in vertebral body was further analyzed. Finally, a feasible scheme was provided for clinical research and analysis. 32 cases of thoracolumbar fractures were selected and treated with open reduction and bone graft fixation. After fixation, the CT density of the anterior and posterior bone graft area was measured at 1 week and 6 months later, as well as the compression ratio of anterior and posterior margin and the Cobb angle of the injured vertebra. The results showed that for patients with high and low density of thoracolumbar fracture after fixation, vertebral compression rate and Cobb value of the wound margin after one month decreased significantly compared with the preoperative, and had statistical significance. However, the value was slightly higher in the final visit after 6 months, but without statistical significance, indicating that patients were in the postoperative recovery while during the recovery period, there was no significant change, also no secondary damage. In addition, the compression ratio and the Cobb value of the anterior and posterior margin of the injured vertebrae in the two groups were basically consistent, indicating the same effect. The relative density of CT in the bone graft area of the patients in the high and low density group at the last visit was significantly higher than that in1 week after operation, indicating that the bone density increased gradually and the bone mass increased gradually.The density of bone graft in the low density group was higher than that in the high-density group at 1 week after the operation and at the last follow-up, but there was no significant difference in the relative density of CT between the low-density group and the high density group at the last follow-up, and the value was close. Therefore, the fracture healing could be evaluated according to the relative density of CT in the patients before and after operation, which might provide the clinical guidance for the patients with thoracolumbar fracture.
