摘要
癌症是威胁人类健康的主要杀手之一。虽然传统微秒脉冲不可逆电穿孔治疗肿瘤方法以其非热效应的优势,在临床实体瘤治疗中得到了大量的应用并取得了良好的治疗效果,但是随着临床研究的深入,发现在治疗过程中仍存在电场分布不均致肿瘤残留以及肌肉收缩等亟需解决的问题。临床需求催生并促进了新一代复合脉冲不可逆电穿孔治疗肿瘤方法的发展与成熟。在简要叙述传统不可逆电穿孔发展历程的基础上,指出目前不可逆电穿孔临床应用存在的相关问题,并针对这些问题引出复合脉冲不可逆电穿孔的肿瘤新型治疗方法;重点介绍了复合脉冲不可逆电穿孔的发展历程及研究成果;以及由我国研究人员独立研发并成功开展的新型复合脉冲不可逆电穿孔治疗前列腺癌的世界首批临床实验,验证了复合脉冲临床应用的安全性与有效性。最后指出了不可逆电穿孔疗法的应用前景和发展趋势。
Tumor becomes one of the major killers to human health. Although the conventional microsecond pulse irre- versible electroporation has been widely applied in solid tumor therapy and achieved great effects because of the non-thermal ablation, some problems still need to be solved with the clinical process, such as incomplete ablation due to the non-uniform distribution of electric field and muscle contraction during treatment. The clinical demands give rise to a new generation irreversible electroporation----composite pulse irreversible electroporation. In this paper, we firstly intro- duced the development of the traditional irreversible electroporation and its problems, then the appearance of composite pulse, especially, the development and the achievement of composite pulse were described. Secondly, the design of the composite pulse apparatus and the first clinical trial on human prostate in the world by Chinese researchers were men- tioned, which verified the safety and efficiency of composite pulse in tumor ablation. Finally, we pointed out the application prospect and development tendency of composite pulse irreversible electroporation.
