摘要
目的:探讨医疗失效模式和效应分析(HFMEA)在儿童静脉用药调配中心(PIVAS)的应用效果。方法:采用HFMEA工作模式,成立HFMEA小组,创建儿童PIVAS静脉用药配置流程,分析过程中的失效模式并评估其风险,分析失效模式的原因并提出针对性的控制方案并执行,比较HFMEA实施前后儿童PIVAS静脉配置流程中差错发生率。结果:HFMEA实施后,儿童PIVAS静脉用药配置流程中各环节差错发生率明显降低,差异有统计学意义(P<0.01)。结论:HFMEA可有效识别儿童PIVAS静脉用药配置流程中的失效模式,分析原因并制定针对性控制方案,从而降低药物配置过程中差错发生率,为临床用药提供了安全保障。
Objective:To explore the application effect of healthcare failure mode and effect analysis(HFMEA)in children pharmacy intravenous admixture services(PIVAS). Methods:Using HFMEA mode,the HFMEA group was established to create the procedure of administration of intravenous drugs. The potential risk of failure mode in this process was analyzed and the reasonable control scheme was proposed and implemented. The incidence of errors in the drug allocation process of children PIVAS was compared before and after the implement of HFMEA. Result:After the implement of HFMEA,the incidence of errors decreased significantly(P<0.05). Conclusion:Application of HFMEA can effectively identify the failure mode in children PIVAS,decrease the incidence of errors in drug allocation process,and provide security for its clinical drug use.
