摘要
目的探讨Swanson人工关节治疗严重拇外翻、拇僵直、Freiberg病及创伤性关节炎的近期疗效。方法采用Swanson可屈曲铰链式人工关节对14例晚期足踝关节疾病的患者(15足)实施人工跖趾关节置换手术。采用AOFAS评分系统对手术前后疼痛、行走、穿鞋及跖趾关节活动度等进行临床评价。结果患者均获得随访,时间3~25个月。术后11足疼痛基本消失,2足疼痛明显缓解,2足疼痛有所改善。患者均未出现假体断裂、松动或骨髓炎。术后患者满意度评价:10例非常满意,2例满意,2例不满意,满意率为12/14。AOFAS评分由术前48.06分±6.78分提高至术后82.45分±5.21分,差异有统计学意义(t=-29.037,P<0.05)。结论 Swanson人工关节置换术治疗严重拇外翻、拇僵直及Freiberg病等晚期足踝关节疾病近期疗效较好。
Objective To investigate the short-term outcome of metatarsophalangeal arthroplasty with silicon Swanson prosthesis for treatment of severe hallux valgus , hallux stiffness , Freiberg disease and traumatic arthritis.Methods Metatarsophalangeal joint arthroplasty with silicon Swanson prosthesis were used to treat 14 cases of terminal foot and ankle joint disease（15 feet）.The AOFAS score system was used to evaluate pain , walking, shoes and metatarsopa-langeal joint range before and after surgery.Results The 14 patients were followed-up for 3~25 months.Pain were disappeared in 11 feet, obviously relieved in 2 feet, and improved in 2 feet.X-ray examination showed no prosthesis fracture, loosening, or osteomyelitis occurred at last follow-up.Postoperative satisfaction evaluation：10 cases were very satisfactory, 2 cases were satisfactory, 2 cases were unsatisfactory, the satisfaction rate was 12/14.The average preoperative AOFAS score was（48.06 ±6.78） points,and improved to （82.45 ±5.21） points postopetatively.Compared with the preoperative AOFAS score , the difference was statistically significant （t=-29.037,P〈0.05）.Conclusions The short-term result of total metatarsophalangeal arthroplasty with silicon Swanson prosthesis for treatment of severe hallux valgus , hallux stiffness , Freiberg disease and traumatic arthritis is satisfied.
