摘要
目的观察痰湿质在阻塞性睡眠呼吸暂停综合征(OSAS)中对生化指标及细胞因子水平的影响。方法将360例OSAS患者按照中医体质标准分为痰湿质组125例、非痰湿质组118例、平和质组117例。3组入院后分别进行整夜多导睡眠监测,计算睡眠呼吸暂停低通气指数(AHI),评估OSAS严重程度;测定3组生化指标高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、极低密度脂蛋白胆固醇(VLDL-C)、总胆固醇(TC)及甘油三酯(TG);测定3组细胞因子脂联素(APN)、细胞黏附分子-1(ICAM-1)、肿瘤坏死因子α(TNF-α)、白细胞介素6(IL-6)及瘦素(LP)。结果 3组病情程度比较差异均有统计学意义(P<0.05),痰湿质组的病情严重程度较重。3组TG、TC水平比较差异有统计学意义(P<0.05),HDL-C、LDL-C、VLDL-C水平比较差异均无统计学意义(P>0.05)。3组APN、ICAM-1、TNF-α、LP比较差异有统计学意义(P<0.05),IL-6比较差异无统计学意义(P>0.05)。结论中医痰湿质患者患OSAS的严重程度较重,同时对生化指标及细胞因子水平有一定影响。
Objective To observe the effects of the phlegm-dampness constitution on biochemical indexes and the level of cytokines in obstructive sleep apnea syndrome( OSAS). Methods 360 cases with OSAS were divided into phlegm-dampness constitution group( n = 125),non-phlegm-dampness constitution group( n = 118)and mild constitutim group( n = 117) according to the constitution standard of traditional Chinese Medicine. After admission,three groups were monitored with polysomnography during the whole night,calculated the apnea-hypopnea index( AHI),and evaluated the severity of OSAS. The biochemical indexes including high-density lipoprotein cholesterol( HDL-C),low-density lipoprotein cholesterol( LDL-C),very low-density lipoprotein cholesterol( VLDL-C),total cholesterol( TC) and triglyceride( TG) were detected in three groups. The cytokines including adiponectin( APN),intercellular cell adhesion molecule-1( ICAM-1),tumor necrosis factor-α( TNF-α),interleukin-6( IL-6) and leptin( LP) were detected in three groups. Results There were statistical differences on the illness severity( P < 0. 05),and the illness severity in phlegm-dampness constitution group was more serious.There were statistical differences on the levels of TG and TC among three groups( P < 0. 05),and with no statistical difference on the levels of HDL-C,LDL-C and VLDL-C( P > 0. 05). There were statistical differences on APN,ICAM-1,TNF-α and LP among three groups( P < 0. 05),and with no statistical difference on IL-6( P > 0. 05).Conclusion The illness severity of OSAS patients with phlegm-dampness constitution is more serious,and the phlegm-dampness constitution has some effects on biochemical indexes and the level of cytokine.
