摘要
目的分析不同分期2型糖尿病肾病患者的血浆同型半胱氨酸(Hcy)水平及其临床意义。方法将96例2型糖尿病肾病患者分为A组(正常白蛋白尿期)30例、B组(早期糖尿病肾病)30例、C组(临床糖尿病肾病期)36例。比较3组血浆Hcy、P-选择素、血浆假血管血友病因子(vWF)及纤维蛋白原水平。分析患者血浆Hcy水平与P-选择素、vWF及纤维蛋白原水平的相关性。结果 C组的Hcy水平、vWF、纤维蛋白原水平高于A组及B组(P<0.05),C组的P-选择素高于A组(P<0.05)。糖尿病肾病患者的Hcy水平与P-选择素、vWF及纤维蛋白原水平均呈正相关(P<0.05)。结论 Hcy可能在促进糖尿病肾病发展中有重要作用,这可能与高同型半胱氨酸血症造成内皮功能障碍致肾脏损害有关。
Objective To analyze the plasma homocysteine( Hcy) levels of patients with type 2 diabetic nephropathy in different stages and its clinical significance. Methods Ninety-six patients with type 2 diabetic nephropathy were divided into group A( normal albuminuria stage,n = 30),group B( early diabetic nephropathy stage,n = 30) and group C( clinical diabetic nephropathy stage,n = 36).The levels of plasma Hcy,P-selectin,von Willebrand's factor( v WF) and fibrinogen were compared among the three groups. The correlation of plasma Hcy level with P-selectin,v WF or fibrinogen level was analyzed. Results The Hcy,v WF and fibrinogen levels of group C were higher than those of group A and group B( P〈0.05). The P-selectin level of group C was higher than that of group A( P〈0.05). The Hcy level positively correlated with P-selectin,v WF and fibrinogen levels in the patients with diabetic nephropathy( P〈0.05). Conclusion Hcy may play an important role in promoting the progression of diabetic nephropathy,which probably relates to endothelial dysfunction-induced renal injury due to hyperhomecysteinemia.
