摘要
目的探讨呼吸训练器进行呼吸功能锻练在心胸外科手术患者的应用效果。方法选取2013年1月1日-2016年12月31日在医院心胸外科住院手术的患者,将160例患者采用随机数字表法分为研究组和对照组各80例。对照组开展腹式呼吸及缩唇呼吸训练,观察组在此基础上采用呼吸训练器开展呼吸功能训练。于手术前及手术后第14天评定两组患者肺功能,统计住院时间及术后并发症发生率。结果对照组和观察组用力肺活量(FVC)[(3.82±0.12)L和(4.46±0.56)L,P<0.01]、1 S呼气量(FEV1)[(3.04±0.45)L和(3.96±0.87)L,P<0.01]、最大深吸气量(IC)[(56.40±3.90)L和(64.45±4.36)L,P<0.01]及住院天数[(16.14±2.18)d和(21.06±3.69)d,P<0.01]差异有统计学意义;观察组无肺不张发生,肺部感染率为2.50%,显著低于对照组(12.5%、6.25%),P<0.01,差异有统计学意义。结论采用呼吸训练器实施呼吸功能锻炼,有助于改善心胸外科手术患者围手术期肺功能,降低术后并发症发生率。
Objective To explore the effect of respiratory training apparatus on respiratory function exercise in cardiothoracic surgery patients. Methods Inpatients in cardiothoracic surgery department of our hospitalfrom January 1, 2013 to December 31, 2016 were selected. 160 patients were randomly divided into the study group and the control group with 80 cases in each group. The control group was given abdominal breathing and pursed lips breathing training and the observation group was given respiratory training based on the breathing training apparatus.The pulmonary function of the two groups was evaluated before operation and fourteenth days after operation, and the length of stay and the incidence of postoperative complications were counted.Results The control group and the observation group(FVC) [(3.82±0.12) L and(4.46±0.56) L,P0.01],(FEV1) [(3.04±0.45) L and(3.96±0.87) L,P0.01],(IC) [(56.40±3.90) L and(64.45±4.36) L,P0.01], Hospitalization days [(16.14±2.18)d and(21.06±3.69)d,P0.01]. The difference was statistically significant. No atelectasis occurred in the observation group. The pulmonary infection rate was 2.50%, which was significantly lower than that of the control group(12.5%, 6.25%),P0.01.The difference was statistically significant. Conclusion Respiratory exercise can improve the pulmonary function and reduce the incidence of postoperative complications in patients undergoing cardiothoracic surgery.
