摘要
目的分析甲状腺疾病住院患者的构成及支付情况,研究甲状腺疾病住院费用特征及影响因素,为医院合理控制甲状腺住院费用提供依据。方法按照国际疾病分类标准(ICD-10),筛选某院2016年1月1日-2016年12月31日期间住院治疗的甲状腺疾病患者病案首页资料共1113例,对住院患者基本情况进行分析,采用卡方检验或方差分析;对住院费用情况进行描述性分析及多重线性回归分析。结果非毒性甲状腺肿出院患者居多,占全部出院人数37.92%,甲状腺恶性肿瘤次之,占29.74%;女性患者高于男性,占全部病例的79.60%(剔除妊娠期甲状腺疾病患者),并多集中在40岁~60岁。参加城镇医疗保险及新型农村合作医疗的患者多于自费患者,占出院患者的86.88%。住院费用构成比顺位为:手术治疗费、西药费、实验室诊断费、耗材类费用、中成药费、一般医疗服务费,住院费用中位数分别为5332.0元、3111.6元、1534.5元、1540.9元、882.1元及866.0元。结论甲状腺疾病重点防治人群为中年女性,不同的甲状腺病种、年龄、性别,不同医疗付费方式、住院时间均会使甲状腺疾病住院费用产生很大变化。
Objective To analyze the composition of hospitalized patients with thyroid diseases and its payment methods, study the characteristics of hospitalization expenses and its influencing factors to provide a basis for reducing the medical costs. Methods According to the International Standard of Disease Classification（ICD-10）,searching the medical records of 1113 inpatients with thyroid diseases in one third-level grade A hospital from January to December 2016, analyze the basic information of inpatient by using constituent ratio, analyze the hospitalization cost by using the descriptive analysis, Chi-square test、Variance analysis and Multiple linear regression. Results Non toxic goiter discharged patients were accounted for 37.92% of all, and malignant thyroid tumors were accounted for 29.74%. The ratio of male and female was 1： 4, and mostly at the age of 41-60.Inpatients in urban medical insurance（urban health insurance） and new rural cooperative medical service（NCMS）were accounted for 86.88% of discharged patients, much more than patients at their own expense. The rank of hospitalization expenses was as follows： surgical treatment fee, western medicine fee, laboratory diagnosis fee,consumable expense, Chinese patent medicine fee and general medical service fee. The median was 5332 yuan,3111.6 yuan, 1534.5 yuan, 1540.9 yuan, 882.1 yuan and 866 yuan respectively. Conclusion Middle aged women are the target of thyroid diseases. Different thyroid diseases, age, gender of patient, different medical payment methods and length of stay affected the hospitalization expenses of thyroid diseases.
