摘要
目的了解我国基层医疗机构医院感染管理现状及存在的问题,为加强我国基层医疗机构医院感染管理薄弱环节提供科学依据。方法通过2016年网络问卷调查法,通过社区卫生服务中心(站)和乡镇卫生院的数据,分析我国基层医疗机构医院感染管理情况。结果经过数据筛查和整理,进入到本次分析范围有479所基层医疗机构,覆盖我国29个省(自治区、直辖市)。353所占73.70%无床位或床位少于50张,428所占89.35%建立了医院感染管理体系,448所占93.53%开展院内医院感染管理培训,282所占58.87%开展了医院感染监测,427所占89.14%开展了手卫生培训工作,399所占83.30%开展了安全注射工作,473所占98.75%开展了医疗废物管理工作,303所占63.26%表示了解院感质控中心的工作。结论国家层面管理政策的制定对规范基层医疗机构的医院感染管理起到重要的作用;基层医疗机构仍需夯实基础,加强临床医师的医院感染防控基础知识培训,提高医院感染监测信息化水平,依法依规开展诊疗服务。同时应加强质控体系建设,加大基层医疗机构质控力度。
OBJECTIVE To understand the current situation and problems of nosocomial infection management in grass-roots medical institutions,so as to provide scientific base to enforcing weak link management.METHODS Using data of community health service centers and township health centers from surveys on 2016 by network questionnaires,the status of nosocomial infection management in grass-roots medical institutions was analyzed.RESULTS The eligible data were 479 institutions covering 29 provinces,autonomous regions and municipalities,in which 353（73.70%）had no bed or less than 50 beds,428（89.35%）had established the hospital infection management system,448（93.53%）had hospital-based nosocomial infection management training,282（58.87%）carried out the nosocomial infection monitoring,427（89.14%）performed hand hygiene training,399（83.30%）carried out the safe injection,473（98.75%）carried out the medical waste management,and 303（63.26%）knew the work of nosocomial infection quality management and control centers.CONCLUSION The formulation of national level management policy plays an important role in regulating the nosocomial infections management of grass-roots medical institutions.The grass-roots medical institutions need to consolidate the foundation,strengthen the basic knowledge training of nosocomial infection prevention and control,improve the level of informative monitoring on nosocomial infections,and carry out diagnosis and treatments according to law.At the same time,the quality control system of nosocomial infections management should be strengthened and give more attention on grass-roots medical institutions.
