加味八珍汤治疗髋关节单纯性滑膜炎气滞血瘀证的临床研究被引量：4

Clinical Study of Jiawei Bazhen Decoction(加味八珍汤) in Simple Synovitis of Hip Joint with Qi Stagnation Blood Stasis

摘要目的:观察加味八珍汤治疗髋关节单纯性滑膜炎气滞血瘀证的临床效果。方法:将138例髋关节单纯性滑膜炎气滞血瘀证患儿随机分为治疗组和对照组,每组69例。对照组给予常规临床治疗,治疗组加服加味八珍汤治疗。治疗2周后,比较两组治疗后基本临床指标(疼痛缓解时间、内展和外旋消失时间、住院时间)、髋关节功能(Harris评分)、疼痛程度(VAS评分)的改善情况,并评估临床疗效,统计两组患者不良反应发生情况及临床复发状况。结果:治疗组治疗后总有效率为92.8%,显著优于对照组的81.2%,差异均有统计学意义(P<0.05);治疗组治疗前疼痛缓解时间,内展、外旋消失时间及住院时间与对照组比较,差异无统计学意义(P>0.05),治疗后,治疗组上述临床指标均显著优于对照组,差异有统计学意义(P<0.05);治疗组治疗前Harris评分、VAS评分与对照组比较,差异无统计学意义(P>0.05),治疗后,治疗组Harris评分显著高于对照组,VAS评分显著低于对照组,差异有统计学意义(P<0.05);随访6个月,治疗组治愈病例复发率为0,显著低于对照组的11.1%,差异有统计学意义(P<0.05)。结论:加味八珍汤治疗髋关节单纯性滑膜炎气滞血瘀证,能够明显改善患者髋关节功能,缓解患髋关节疼痛,快速改善患儿临床症状,安全可靠。 Objective： To observe the clinical effect of Jiawei Bazhen decoction in the treatment of simple synovitis of hip joint with Qi stagnation blood stasis. Methods： A total of 138 children with simple synovitis of hip joint with Qi stagnation blood stasis were randomly divided into the treatment and control groups, with 69 cases in each group. The patients were treated by routine treatment in control group and oral Jiawei Bazhen de- coction in the treatment group. After 2 weeks of treatment, the basic clinical indicators （pain relief time, dura- tion of internal and external rotation, hospitalization time）, hip function （Harris score） and degree of pain （VAS score） were compared between the two groups, and the adverse reactions and the clinical recurrence were counted. Results： The total effective rate was 92.8% in treatment group and 81.2% in control group, and the treatment group showed better efficacy than the control group, with statistically significant difference （P〈0.05）; Be- fore treatment, the pain relief time, internal and external rotation show disappeared time and hospitalization time between the two groups was not statistically significant （P〉0.05）. After the treatment, the clinical indicators of the treatment group were significantly better than those of the control group, and the difference was statistically sig- nificant （P〈0.05）; Before treatment, the Harris score, VAS score between the two groups were not statistically sig- nificant （P〉0.05）. After treatment, the Harris score of the observation group was significantly higher than the control group, and the VAS score was significantly lower than the control group （P〈0.05）; After 6 months of fol- low-up, there was no recurrence in the treatment group, which was significantly lower than the control group of 11.1% （P〈0.05）. Conclusion： The application of Jiawei Bazhen decoction in the treatment of simple synovitis of hip joint with Qi stagnation blood stasis can significantly improve the hip joint function, relieve hip joint pain,and rapidly improve the clinical symptoms. It is safe and reliable.
