非酒精性脂肪肝病患者肝脏脂肪的磁共振定量研究 被引量:14

Quantitative MRI of Hepatic Fat Fraction in NAFLD Patients

摘要 目的探讨MRI非对称回波三点法水脂分离-定量测定(IDEAL-IQ)技术对非酒精性脂肪肝病(NAFLD)定量诊断及分度的应用价值。方法将超声(US)检查诊断的68例NAFLD患者及50例对照者分别行3.0 T MRI IDEAL-IQ技术定量检查,测定肝脏脂肪含量,再将其分为轻度、中度、重度NAFLD组及对照组。分析US与IDEALIQ技术对NAFLD诊断及分度的一致性。结果 US诊断NAFLD患者68例(轻度36例、中度26例、重度6例),对照者50例;MRI IDEAL-IQ技术诊断NAFLD患者67例(轻度34例、中度30例、重度3例),对照者51例。二者对NAFLD的诊断及分度具有一致性(Kappa值=0.76,P<0.05)。结论 MRI IDEAL-IQ技术对NAFLD的诊断及分度与US一致,能为NAFLD提供定量影像学指标。 Objective To explore the application value of MRI with IDEAL-IQ(iterative decomposition of water and fat with echo asymmetry and least squares estimation image quantification) sequence on quantitative diagnosis and grading in non-alcoholic fatty liver disease(NAFLD) patients. Methods 68 NAFLD patients and 50 healthy controls with normal liver were diagnosed by ultrasonography(US). The NAFLD patients were divided into mild,moderate,and severe NAFLD group. Hepati fat fraction was then quantitatively measured by IDEAL-IQ sequence. All the subjects were divided into mild,moderate and severe NAFLD group and the control group. Statistical Package for the Social Sciences(SPSS) 19. 0 software was used to analyze the agreement on diagnosis and grading between US and IDEAL-IQ sequence(P〈0. 001).Results 68 NAFLD patients(36 mild subjects,26 moderate subjects and 6 severe subjects) and 50 control subjects with normal liver were diagnosed by US. 67 NAFLD patients(34 mild subjects,30 moderate subjects and 3 severe subjects) and51control subjects with normal liver were diagnosed by IDEAL-IQ sequence. IDEAL-IQ sequence and US have consistency with each other in diagnosing and grading NAFLD(Kappa = 0. 76,P〈0. 001). Conclusion IDEAL-IQ sequence can diagnose and grade NAFLD and has agreement with US. IDEAL-IQ sequence might provide an objective and quantitative imaging index for NAFLD.
