摘要
目的 研究男方药物干预联合女方异体淋巴细胞免疫治疗对习惯性流产的治疗作用。方法 选择封闭抗体阴性习惯性流产患者350例,按照干预措施随机分成三组,A组:男方口服左卡尼汀+维生素E+辅酶Q10,女方行异体淋巴细胞细胞免疫治疗并再次妊娠后给予常规保胎治疗;B组:男方无干预措施,女方行异体淋巴细胞免疫治疗并再次妊娠后给予常规保胎治疗;C组:仅在女方再次妊娠后给予常规保胎治疗。比较各组治疗后男方精子相关指标、女方封闭抗体转阳率和再次妊娠后流产率。结果 A组治疗后精子活力高于B组和C组,精子畸形率及DNA碎片率低于B组和C组;A组和B组的封闭抗体转阳率高于C组。A组妊娠后再次流产率最低。结论 淋巴细胞免疫治疗是一种有效的治疗习惯性流产的方法,男性药物干预可以帮助改善习惯性流产患者的妊娠结局,其机制可能是通过改善精子活力、形态和DNA损伤来发挥作用。
Objective To study the therapeutical effect of pharmacological intervention for husbands combined with immunotherapy by lymphocytes of others for wives on habitual abortion.Methods A total of 350 patients with habitual abortion who were lack of blocking antibody were randomly divided into three groups. Group A: the husbands were treated with L-Carnitin, vitamin E and coenzyme, and the wives were treated with immunotherapy by lymphocytes of others and underwent conventional therapy to prevent miscarriage after pregnancy. Group B: the husbands were treated with no intervention, and the wives were treated with immunotherapy by lymphocytes of others and underwent conventional therapy to prevent miscarriage after pregnancy. Group C: only the wives were treated with conventional therapy to prevent miscarriage after pregnancy. The sperm parameters, blocking antibody conversation rate and abortion rate of each group after re-pregnancy were compared.Results After intervention, the sperm motility of group A was higher than that of group B and group C. The sperm deformity rate and DNA fragments rate of group A were both lower than those of group B and group C. The blocking antibody conversation rate of group A and group B was both higher than that of group C. The abortion rate of group A was the lowest among the three groups.Conclusions Immunotherapy by lymphocytes is effective on habitual abortion, pharmacological intervention for husbands can help to improve the pregnancy results, and the mechanism maybe improving the sperm motility, sperm morphology and sperm DNA damage.
