摘要
目的探究电针刺激结合Bobath技术对脑卒中患者下肢运动功能的治疗效果。方法选取我院脑卒中患者87例作为研究对象,采用随机数字表法将患者分为A组、B组、C组,每组29例,A组患者采用Bobath技术培训,B组患者采用电针刺激进行治疗,C组在进行Bobath技术培训基础上结合电针刺激治疗。采用Fugl-Meyer运动功能评分(FMA)、改良Barthel指数评分(MBI)、改良Ashworth分级(MAS)、步行分级评定(FAC)对治疗前后患者下肢的运动情况进行评定。结果 3组患者比较,治疗前FMA、MBI评分各项以及整体评分的差异均无统计学意义(P>0.05)。治疗后3组患者的FMA、MBI评分各项以及整体评分均高于治疗前,且治疗后C组FMA、MBI评分各项以及整体评分高于A、B组,差异均有统计学意义(P<0.05)。3组患者比较,治疗前MAS分级、FAC分级的差异无统计学意义(P>0.05),治疗后C组与A、B组比较,MAS分级的差异有统计学意义(P<0.05),C组与A组比较FAC分级的差异有统计学意义(P>0.05),C组与B组比较FAC分级的差异无统计学意义(P>0.05)。结论电针刺激结合Bobath技术对脑卒中患者下肢运动功能的治疗效果较好,与单纯进行电针刺激或Bobath技术培训相比,能够明显提高患者日常生活能力、下肢运动功能、步行能力以及改善肢体痉挛状况。
Objective To determine the therapeutic effect of electroacupuncture combined with the Bobath technique on the lower limb motor function in stroke patients. Methods Eighty-seven patients with stroke were included and randomly divided into groups A, B, and C (n = 29 in each group). The patients were treated with the Bobath technique in group A, electroacupuncture in group B, and elec-troacupuncture combined with the Bobath technique in group C. The Fugl-meyer Assessment ( FMA ) , Modified Barthel Index ( MBI ), Motor Assessment Scale ( MAS ), and Functional Ambulation Category ( FAC ) were used to assess lower limb movement before and after treatment. Results Before treatment,no significant differences in FMA and MBI scores were found among the three groups (P 〉 0.05). After treatment, the FMA and MBI scores in the three groups were significantly higher than those before the treatment, and the scores in group C were significantly higher than those in groups A and B (P 〈 0.05). No significant differences in MAS and FAC grading were found among the three groups before treatment (P 〉 0.05). After treatment, there were significant differences in MAS grading between group C and groups A and B and FAC grading between group C and group A (P 〈 0.05). No significant difference in FAC grading was found between group C and group B (P 〉 0.05). Conclusion The recovery of lower limb motor function was better in the stroke patients treated with electroacupuncture combined with the Bobath technique, which can significantly improve the ability of daily living,lower limb motor function, walking ability, and limb spasticity, compared with the simple electroacupuncture or Bobath technique.
