摘要
目的:探讨分段双重挂线术联合中药熏洗对高位肛瘘的疗效及并发症的影响。方法:选取126例高位肛瘘患者,随机分为对照组和治疗组。两组均予分段双重挂线术及常规术后治疗,治疗组再联合中药熏洗。观察两组术后1 d及术后4周VAS疼痛评分、创面分泌物评分,术后8周通过Wexner评分、肛门功能指诊评分评估肛门功能;同时观察治疗过程中并发症情况,评估临床疗效。结果:总有效率对照组为77.8%,治疗组95.2%,两组总有效率比较,差异有统计学意义(P<0.01)。两组术后4周VAS疼痛评分、创面分泌物评分均低于同组术后1 d(P<0.05);术后4周治疗组VAS疼痛评分、创面分泌物情况评分均低于同期对照组(P<0.05)。术后8周治疗组Wexner评分、肛门功能指诊评分均低于对照组(P<0.05)。术后28 d,治疗组流脓、瘙痒明显改善情况均优于对照组(P<0.05),而未改善情况均明显低于对照组(P<0.01)。结论:分段双重挂线术联合中药熏洗治疗高位肛瘘,能有效减轻患者术后疼痛及创面分泌物,促进肛门功能恢复,提高临床疗效,对术后流脓、瘙痒及感染等并发症改善满意。
Objective： To investigate the influence of subsection double thread drawing therapy with tradi- tional Chinese medicine fumigation curative effect and complications on high anal fistula. Methods： A total of 126 cases of patients of high anal fistula were selected and randomly divided into control and treatment groups. The patients were treated with segmental double thread incision and routine operation in the two groups, and the treatment group combined with traditional Chinese medicine fumigation. The VAS score and wound secretion score were observed in two groups in one day and 4 weeks after operation; the anal function was evaluated ac- cording Wexner score and anorectal function examination score, in 8 weeks after operation. At the same time, the complications, clinical evaluation were observed. Results： The total effective rate was 77.8% in the control group and 95.2% in the treatment group, and there was significant difference on the total effective rates between the two groups （P〈0.01）. After 4 weeks, the two groups showed lower VAS pain score and wound secretion scores than the same group after one day （P〈0.05）; The treatment group showed lower VAS pain score and wound secretion scores than than the control group 4 weeks after operation （P〈0.05）. The treatment group showed lower Wexner score and anorectal function examination score than than the control group 8 weeks after operation （P〈0.05）. After 28 days, the treatment group had significantly improved purulence and itch, significantly better than the control group （P〈0.05）, while no improvement was significantly lower than the control group （P〈0.01）. Conclusion： The segmental double thread drawing therapy combined with herbal fumigation treatment of high anal fistula, can effectively reduce postoperative pain and wound secretion, promote the recovery of anal function, improve clinical efficacy, to improve the itching, itching and infection complications such as postoperative satisfaction.
