神经导航钻孔引流联合尿激酶治疗基底节脑出血的效果分析 被引量:4

摘要 目的探讨神经导航钻孔引流术联合尿激酶治疗基底节脑出血的效果。方法回顾2015年1月~2017年1月我科神经导航下颅骨钻孔引流术后给予尿激酶治疗的36例高血压基底节脑出血患者的临床资料,对其手术效果、并发症、临床预后进行评价总结。结果纳入研究的36例患者,2例死于呼吸衰竭,1例死于心脏疾病,另有5例患者出现手术相关并发症,包括引流管异位1例,血肿腔再出血2例,穿刺道出血1例,颅内积气1例,随访观察3个月,有22例的患者取得较好的治疗效果(格拉斯哥预后评分>3分)。结论神经导航下钻孔引流术后给予尿激酶治疗基底节脑出血是简单、安全有效的治疗方案。 Objective To explore the effect of a combination of neuronavigation-assisted minimally invasive operation and Urokinase on the treatment of hypertensive basal ganglia hemorrhage.Methods A retrospective analysis was performed by including 36 patients with hypertensive basal ganglia hemorrhage that were treated by Urokinase following neuronavigation-assisted minimally invasive operation from January 2015 to January 2017 in our hospital.Surgical re- suits,procedure-related complications and clinical outcomes were evaluated subsequently.Results A total of 36 patients were enrolled in the study,2 patients died of respiratory failure (two patients) and 1 patient died of heart disease.In addition,5 patients occurred surgical complications,including 1 case of ectopie drainage,2 cases of rebleeding into a hematoma cavity,1 case of hemorrhage in puncture tract,and 1 case of intracranial pneumatocele.Incorporated patients were followed up for 3 months,22 patients achieved better treatment results (Glasgow outcome scale score 〉 3 scores). Conclusion Neuronavigation-assisted minimally invasive operation and subsequent fibrinolysis using Urokinase is a simple,safe and effective procedure for the treatment of hypertensive basal ganglia hemorrhage.
