摘要
目的探讨经脐单孔腹腔镜联合胃镜治疗胃间质瘤的临床价值。方法 2011年1月~2016年3月我科对21例胃间质瘤行经脐单孔腹腔镜联合胃镜手术切除,术中胃镜定位胃间质瘤的位置,以肿瘤中心,距肿瘤周围2 cm处用60mm直线型切割闭合器(强生EC60)切除胃间质瘤。结果 21例手术均获成功,无中转开腹和手术死亡病例。手术时间30~120 min(平均49 min);术中出血量30~100 ml(平均46 ml);术后肛门排气时间7~25 h(平均11 h);术后5~12 d(平均7 d)出院。无术后出血、胃漏等并发症发生。术后20例随访(随访率95.2%)3个月~5年,中位随访时间4.2年,无肿瘤复发、远处转移、死亡。结论经脐单孔腹腔镜联合胃镜治疗胃间质瘤效果确切,安全、微创,有临床推广价值。
Objective To explore the clinical value of single port laparoscopy combined with gastroscopy in the treatment of gastric stromal tumors. Methods From January 2011 to March 2016,21 cases of gastric stromal tumor underwent transumbilical single port laparoscopic surgery combined with endoscopic resection in our hospital. During the operation,the gastric stromal tumor was located under gastroscopy and then resected with 2 cm margin around away from the tumor with 60 mm linear resection（Johnson EC60）. Results The operations in the 21 cases were successful. No conversion to open surgery or operative mortality was seen. The operation time ranged 30-120 min（mean,49 min）; the amount of intraoperative bleeding was 30-100 ml（mean,46 ml）; the postoperative anal exhaust time was 7-25 h（mean,11 h）; the postoperative hospital stay was 5-12 d（mean,7 d）. No complications after operation such as bleeding or gastric leak occurred. After operation,20 cases were followed up for 3 months to 5 years, and the median follow-up period was 4. 2 years. There was no tumor recurrence, distant metastasis or death.Conclusion Single port laparoscopy combined with gastroscopy in the treatment of gastric stromal tumors is effective,safe and minimally invasive,being worthy of further clinical promotion.
