多层面螺旋CT三维重建在诊断良性和恶性脊柱骨折中的价值 被引量:4

The value of multi-slice spiral CT three-dimensional reconstruction in the diagnosis of benign and malignant spinal fractures

摘要 目的探讨多层面螺旋CT三维重建在诊断良性和恶性脊柱骨折中的价值。方法选取本院2014年10月~2016年10月诊治的良性脊柱骨折患者51例为对照组,恶性脊柱骨折患者51例为观察组,均行多层面螺旋CT三维重建,比较两组患者的图像差异和诊断价值。结果图像指标分析中,观察组患者ADCla、ADCli、ADCle、da、di、de低于对照组(P<0.05)。图像表征分析中,观察组患者T2W1高信号所占比例、椎体形态(倒楔形)所占比例、椎体后缘膨隆所占比例、椎旁软组织结节肿块所占比例高于对照组(P<0.05)。观察组患者椎体形态(楔形)所占比例、椎体后缘成角所占比例、椎旁软组织环形增厚所占比例低于对照组(P<0.05)。与病理结果相比,对照组患者的灵敏度为98.0%,特异度为96.1%,准确度为97.1%。观察组患者的灵敏度为100.0%,特异度为98.0%,准确度为99.0%。两组患者间灵敏度、特异度、准确度比较,差异均无统计学意义(P>0.05)。结论多层面螺旋CT三维重建可用于诊断良性和恶性脊柱骨折,具有高灵敏度、高特异度、高准确度,值得临床推广使用。 Objective To explore the value of multi-slice spiral CT three-dimensional reconstruction in the diagnosis of benign and malignant spinal fractures. Methods A total of 51 patients with benign spinal fractures diagnosed and treated in our hospital from October 2014 to October 2016 were selected as the control group and 51 patients with ma- lignant spinal fractures were selected as the observation group. The patients were treated with multi-slice spiral CT three-dimensional reconstruction. The image differences and diagnostic value between the two groups of patients were compared. Results In the analysis of image indexes, ADCla, ADCli, ADCle, da, di, de were lower in the observation group than those in the control group(P〈0.05). In the image characterization analysis, the proportion of T2W1 high sig-nal, the proportion of vertebral body(inverted wedge), the proportion of posterior vertebral bulging, the proportion of parietal soft tissue nodules in the observation group were higher than those of the control group(P〈0.05). The proportion of vertebral morphology(wedge), vertebral posterior margin angulation and paraspinal soft tissue annular thickening in the observation group were lower than those in the control group (P〈0.05). The sensitivity, the specificity and the accu-racy of pathological results in the control group was 98.0%, 96.1% and 97.1%, respectively. And the sensitivity, the specificity and the accuracy of pathological results in the observation group was 100.0%, 98.0%. and 99.0%, respec- tively. There was no significant difference in sensitivity, specificity and accuracy between the two groups (P〉0.05). Conclusion Multi-slice spiral CT three-dimensional reconstruction can be used to diagnose benign and malignant spinal fractures, which has high sensitivity, high specificity and high accuracy, and is worthy of promotion and clinical use.
