摘要
从团队管理的概念、发展、实施过程、效果等方面综述社区糖尿病团队管理的研究进展,指出糖尿病的团队管理是长期的过程,需要团队成员动态随访、评估病人,制定动态的治疗方案与目标。我国采取糖尿病团队管理模式的时间还很短,团队成员尚无统一的标准,大部分团队的成员就是全科医生和护士。
This article reviewed the research progress of team management community diabetes from the concept,development,process,effect and other aspects of team management,pointed out that the team management of diabetes is a long-term process.Team members were required to follow up and evaluate patients dynamically,to make a dynamic treatment plan and target.The period of team management model applied in diabetes were short in China,and there was no unified standard for team members,most of whom were general practitioners and nurses.
