摘要
目的观察天楼解毒消肿散治疗糖尿病足的可能作用机制。方法将60只大鼠随机分为正常对照组、模型组、天楼解毒消肿散组、莫匹罗星组和如意金黄散组,每组12只。除正常对照组外,其余组大鼠采用高脂饲料喂养加链脲佐菌素液腹腔注射制成糖尿病大鼠模型,用烫伤法制备糖尿病足大鼠模型。正常对照组不予任何处理,模型组每天常规清理患肢溃疡部位,天楼解毒消肿散组予以天楼解毒消肿散直接撒于溃疡处,莫匹罗星组予以莫匹罗星膏外敷,如意金黄散组予以如意金黄散涂抹于患处或用凡士林调和后摊于纱布上贴患处。各组均以覆盖整个溃疡面为度,每日1次。给药2周后观察大鼠创面一般情况及溃疡面积大小变化,HE染色观察各组大鼠足背动脉变化,免疫组化法观察足背动脉缺氧诱导因子1α(HIF-1α)、血管内皮生长因子(VEGF)蛋白表达及微血管密度(MVD)。结果与模型组比较,天楼解毒消肿散组、莫匹罗星组、如意金黄散组溃疡面积明显减小,且天楼解毒消肿散组溃疡面积最小(P<0.05)。与正常对照组比较,模型组HIF-1α、VEGF蛋白表达及MVD明显增强(P<0.05)。与模型组比较,天楼解毒消肿散组、莫匹罗星组、如意金黄散组足背动脉中HIF-1α、VEGF蛋白表达及MVD明显升高(P<0.05),且天楼解毒散组较莫匹罗星组、如意金黄散组升高(P<0.05)。结论天楼解毒消肿散能提高足背动脉中HIF-1α、VEGF蛋白表达及MVD,可能是其治疗糖尿病足的机制之一。
Objective To observe the possible mechanism of Tianlou Jiedu Xiaozhong San( 天楼解毒消肿散,TJXS) in the treatment of diabetic foot. Methods Sixty rats were randomized into control group,model group,TJXS group,mupirocin group and Ruyi Jinhuang San( 如意金黄散,RJS) group,with 12 rats in each group. Diabetic model was made by high fat diet and streptozotocin( i. p.) in each group except the control group. Diabetic foot model was made by burning. The control group was not given any treatment,the model group was given daily routine clearance of limb ulcers,TJXS group was given TJXS scattered directly to the ulcer,mupirocin group was given mupirocin externally applied and RJS group was given RJS externally applied or with Vaseline on the gauze on the affected part. The entire ulcer surface should be just covered in each group and the treatments were given daily. After 2 weeks of administration,the general condition and the size of ulcer were observed,the changes of dorsalis pedis artery in each group were observed by HE staining,and the expressions of hypoxia-inducible factor-1α( HIF-1α),vascular endothelial growth factor( VEGF) protein and microvessel density( MVD) were observed by immunohistochemistry.Results Compared with that of the model group,the ulcer area of TJXS group,mupirocin group,RJS group significantly reduced,and that in TJXS group was the smallest( P〈 0. 05). Compared with those in the control group,the expressions of HIF-1α,VEGF protein and MVD in the model group significantly increased( P 〈0. 05). The expressions of HIF-1α,VEGF protein and MVD in the dorsalis pedis artery in TJXS group,mupirocin group and RJS group were significantly higher than those in the model group( P〈 0. 05),and those in TJXS group were significantly higher than those in mupirocin group and RJS group( P〈 0. 05). Conclusion TJXS could increase the expressions of HIF-1α,VEGF protein and MVD in the dorsalis pedis artery,which may be one of the mechanisms in its treatment for diabetic foot.
