摘要
目的分析重型颅脑损伤(SBI)患者尿路感染病原菌分布及危险因素分析。方法选取医院2014年8月-2016年8月收治的SBI患者156例作为研究对象,对所有纳入患者的术后尿路感染情况进行观察和分析,对与患者术后尿路感染发生相关的因素进行研究和分析,对发生尿路感染患者尿液样本中的病原菌进行检测和分析。结果术后发生尿路感染患者51例,感染率32.69%,感染患者以发热、尿路刺激征、尿道口瘙痒为主要临床体征;共分离病原菌72株,其中革兰阴性菌35株占48.61%,以大肠埃希菌为主;革兰阳性菌11株占15.28%,以粪肠球菌为主,真菌26株占36.11%,以白色假丝酵母菌为主;合并糖尿病、GCS评分、留置导尿管时间是SBI患者发生尿路感染的独立危险因素(P<0.05)。结论 SBI术后尿路感染的相关因素、临床体征和病原菌分布具有特征性,给予充分的重视和有效的诊治,以提高SBI患者救治成功率和医院感染工作的效果。
OBJECTIVE To analyze the distribution of pathogenic bacteria and risk factors of urinary tract infections in patients with severe brain injury（SBI）.METHODS A total of 156 patients with SBI from Aug.2014 to Aug.2016 were selected as the research subjects.The conditions of postoperative urinary tract infections in all the included patients were observed and analyzed.The factors related to postoperative urinary tract infections of patients were investigated and analyzed.The pathogenic bacteria in the urine samples of patients with postoperative urinary tract infections were detected and analyzed.RESULTS Totally 51 cases were observed with urinary tract infections,and the infection rate was 32.69%.The main clinical symptoms of patients with SBI complicated with postoperative urinary tract infections were fever,urinary tract irritation and urethral meatus itching.Totally 72 strains of pathogenic bacteria were isolated,including 35 strains of gram-negative bacteria,accounting for 48.61%,which were mainly Escherichia coli,11 strains of gram-positive bacteria,accounting for 15.28%,which were mainly Enterococcus faecalis,and 26 strains of fungi,accounting for 36.11%,which were mainly Candida albicans.Diabetes,GCS score and indwelling catheter time were independent risk factors for urinary tract infections in SBI patients（P〈0.05）.CONCLUSION The related factors,clinical signs and pathogenic bacteria distribution of patients with SBI complicated with postoperative urinary tract infection have some characteristics.The clinicians should give full attention and effective treatments in order to improve the success rate of treatment of patients with SBI infection and the effects in prevention and control of nosocomial infections.
