摘要
目的探讨非ST抬高急性冠脉综合征患者并发肺部感染的危险因素。方法回顾性分析医院2013年2月-2016年1月101例非ST抬高急性冠脉综合征患者,观察肺部感染发生情况并分析其肺部感染的危险因素。结果 101例患者中肺部感染有9例,发生率8.91%;慢性阻塞性肺疾病、侵入性操作、心功能Killip分级、左室射血分数≤0.4、多壁心肌梗死是肺部感染的相关因素(P<0.05);慢性阻塞性肺疾病、侵入性操作、心功能Killip分级(Ⅲ~Ⅳ级)、左室射血分数≤0.4、多壁心肌梗死是非ST抬高急性冠脉综合征并发肺部感染高独立危险因素(P<0.05)。结论非ST抬高急性冠脉综合征并发肺部感染的危险因素较多,应评估患者的个体情况,制定针对性的预防患者肺部感染的措施,以降低患者肺部感染率,提升患者的预后质量。
OBJECTIVE To investigate the risk factors of pulmonary infections in patients with non ST elevation acute coronary syndrome.METHODS A retrospective analysis of 101 patients with non ST elevation acute coronary syndrome from Feb.2013 to Jan.2016 was conducted to observe the pulmonary infection rate and analyze the risk factors of pulmonary infections.RESULTS There were 9 cases of pulmonary infections in 101 cases,and the incidence rate was 8.91%.With chronic obstructive pulmonary disease,invasive operation,cardiac function（Killip grade Ⅲ~Ⅳ）,left ventricular ejection fraction of 0.4 or less and multi-wall myocardial infarction were risk factors of pulmonary infections（P〈0.05）.Chronic obstructive pulmonary disease,invasive operation,cardiac function（Killip grade Ⅲ~Ⅳ）,left ventricular ejection fraction of 0.4 or less and multi wall myocardial infarction were independent high-risk factors of pulmonary infections in patients with non ST elevation acute coronary syndrome.CONCLUSION There are many risk factors of pulmonary infections in patients with non ST elevation acute coronary syndrome,so we should carefully assess the individual condition of the patient,formulate corrective measures for prevention of pulmonary infection according to risk factors of the patients,so as to reduce the pulmonary infection rate and improve the prognosis of patients with quality.
