老年2型糖尿病患者尿路感染与血糖控制的相关性分析 被引量:17

摘要 目的分析老年2型糖尿病患者尿路感染临床特点和血糖控制情况。方法选取医院2015年11月-2017年1月收治的老年2型糖尿病合并尿路感染患者52例为观察组,同时选取同期在医院治疗的老年2型糖尿病未合并尿路感染患者51例为对照组,分析老年2型糖尿病患者尿路感染临床特点,并对比两组的治疗时间、病程、生化指标情况。结果 52例感染患者共分离培养病原菌52株,其中革兰阴性菌48株占92.31%,以大肠埃希菌为主;真菌4株占7.69%;观察组治疗时间、病程、尿素氮、甘油三酯、糖化血红蛋白、空腹血糖及餐后2h血糖分别为(16.62±2.90)d、(11.34±2.00)年、(6.73±1.23)mmol/L、(2.08±0.70)mmol/L、(14.03±1.80)%、(8.30±1.92)mmol/L、(14.41±2.22)mmol/L均显著高于对照组(12.48±2.50)d、(8.46±2.03)年、(4.67±1.20)mmol/L、(1.40±0.40)mmol/L、(9.64±1.56)%、(7.05±1.17)mmol/L、(11.12±1.90)mmol/L(P<0.05)。结论老年2型糖尿病患者尿路感染病原菌以大肠埃希菌为主,同时患者的各项生化指标、治疗时间、病程均明显升高,血糖控制效果不佳。 OBJECTIVE To analyze the clinical characteristics of urinary tract infections and blood glucose control in elderly patients with type 2 diabetes mellitus.METHODS Fifty-two elderly patients with type 2 diabetes mellitus combined with urinary tract infections who admitted in our hospital from Nov.2015 to Jan.2017 were selected and included into observation group,and fifty-one elderly patients with type 2 diabetes mellitus without urinary tract infection at the same period were selected as control group.The clinical characteristics of urinary tract infections in elderly patients with type 2 diabetes mellitus were analyzed,and the treatment time,course of disease and biochemical indicators were compared between two groups.RESULTS A total of 52 strains of pathogenic bacteria were isolated from 52 cases of infected patients,of which 48 strains were gram-negative bacteria accounting for 92.31%,mainly Escherichia coli,and 4 strains were fungi accouting for 7.69%.The treatment time,course of disease,urea nitrogen,triglyceride,HbA1 c,fasting blood glucose and postprandial 2 hblood glucose of observation group were(16.62±2.90)d,(11.34±2.00)years,(6.73±1.23)mmol/L,(2.08±0.70)mmol/L,(14.03±1.80)%,(8.30±1.92)mmol/L and(14.41±2.22)mmol/L,which were significantly higher than(12.48±2.50)d,(8.46±2.03)years,(4.67±1.20)mmol/L,(1.40±0.40)mmol/L,(9.64±1.56)%,(7.05±1.17)mmol/L and(11.12±1.90)mmol/L of control group(P〈0.05).CONCLUSION The main pathogenic bacteria of urinary tract infections in elderly patients with type 2 diabetes mellitus is E.coli,meanwhile,the biochemical indicators,treatment time and the course of disease are significantly increased,and the blood glucose control effect is poor.
