摘要
目的分析注射用糜蛋白酶在预防鼻肠管堵塞中的临床作用。方法随机选取2015年3月~2016年8月80例普外科腹部手术后留置鼻肠管进行肠内营养的患者作为研究对象,分为治疗组与对照组,每组40例。治疗组采用滴注完营养液后用温热注射用水50m L冲管后,再用注射用糜蛋白酶4000单位加0.9%生理盐水50m L封管,隔日1次;对照组采用营养液滴注完后用温热注射用水50m L冲管。观察两组患者堵管发生率、住院时间、营养不良发生率和血清白蛋白指标的差异。结果治疗组与对照组的堵管发生率分别为7.5%和20.0%、住院时间分别为(12.6±4.2)d和(16.4±7.6)d,观察组在各方面显著优于对照组,两组间的比较差异具有统计学意义(P<0.05)。结论注射用糜蛋白酶能够有效预防鼻肠管堵塞,避免鼻肠管堵塞引起的不良反应,改善患者的预后情况,提高患者的生活质量。
Objective To analyze the clinical effect of Chymotrypsin for Injection in preventing obstruction of nasal tube. Methods 80 cases of patients undergoing enteral nutrition with indwelling nasal tube after abdominal surgery from March 2015 to August 2016 were randomly selected.The patients were divided into treatment group and control group,d0 cases in each group.The patients of treatment group were treated with drip injection of nutrient solution,with warm water injection 5OraL,flushed tube, and then with Chymotrypsin for Injection 4000 units plus 0.9% physiological saline 50ml sealed tube,1 times every other day.The patients of control group were treated with drip injection of nutrient solution,after injection,with warm water injection 50mL,flushed tube.The incidence of blocked tube,hospitalization time,incidence of malnutrition and serum albumin index were observed between the two groups. Results In treatment group and control group,the plugging rate of were 7.5% and 20%,the hospitalization were（12.6 ± 4.2）d and （16.4 ± 7.6）d,all indexes of observation group were significantly better than those of control group,the differences were statistically significant between the two groups（P 〈 0.05）. Conclusion Chymotrypsin for Injection can effectively prevent the obstruction of the nasal duct and avoid the adverse reactions caused by the obstruction of the nasal duct,improve the prognosis of the patients and improve the quality of life of the patients.
