2型糖尿病合并非酒精性脂肪性肝病与体成分、血脂的相关性研究 被引量:6

Study on the correlation between body compositions,blood lipid and nonalcoholic fatty liver disease in patients with type 2 diabetes mellitus

摘要 目的分析2型糖尿病合并非酒精性脂肪性肝病患者的体成分特点及血脂水平,探讨体成分、血脂水平与糖尿病合并脂肪肝发生发展的关系。方法随机选取我院2型糖尿病住院患者111例,其中2型糖尿病合并非酒精性脂肪肝患者63例,单纯2型糖尿病患者48例。采用生物电阻抗方法测得患者体成分包括蛋白质、无机盐、体脂肪、肌肉量、去脂体重、体脂百分比、腰臀比、内脏脂肪面积、肥胖程度、身体细胞量、骨矿物质含量等;同时测定患者血甘油三酯、总胆固醇、高密度脂蛋白、低密度脂蛋白等指标。应用超声检查确定脂肪肝情况,并分析脂肪肝严重程度与体成分、血脂指标的相关性。结果 2型糖尿病合并非酒精性脂肪性肝病患者的体重指数、体脂肪、体脂百分比、腰臀比、内脏脂肪面积、肥胖程度、身体细胞量均明显高于单纯2型糖尿病组(P<0.05);且脂肪肝严重程度与体质指数、体脂肪、体脂百分比、腰臀比、内脏脂肪面积、肥胖程度、血甘油三酯水平等指标均呈正相关(P<0.05)。结论体质指数、体脂肪、体脂百分比、腰臀比、内脏脂肪面积、肥胖程度、血甘油三酯升高明显的患者,要警惕2型糖尿病合并非酒精性脂肪性肝病的发生;体重指数、血甘油三酯水平是2型糖尿病发生非酒精性脂肪性肝病的独立危险因素。 Objective To analyze the body composition and blood lipid levels of patients with type 2 diabetes mellitus and nonalcoholic fatty liver disease, and to explore the relationship between body composition, blood lipid level and the development of nonalcoholic fatty liver disease in patients with type 2 diabetes mellitus. Methods In this study, 111 patients with type 2 diabetes mellitus hospitalized in our hospital were enrolled, including 63 patients with type 2 diabetes and nonalcoholic fatty liver disease, and 48 patients with type 2 diabetes alone. The body composition of the patients were measured by bioelectrical impedance method, including protein, inorganic salt, body fat mass, muscle mass, fat free weight, body fat percentage, waist to hip ratio, visceral fat area, obesity degree, body cell volume, bone mineral content and so on. And the indexes of blood triglyceride, total cholesterol, high density lipoprotein, low density lipoprotein were also measured. The condition of fatty liver was determined by ultrasonography, and the correlation among the severity of fatty liver, body composition and lipid indexes were analyzed. Results Body mass index, body fat mass, body fat percentage, waist to hip ratio, visceral fat area, obesity degree, body cell volume in type 2 diabetic patients with nonalcoholic fatty liver disease were significantly higher than those of type 2 diabetes alone group respectively (P 〈 0.05) . The severity of fatty liver was positively correlated with body mass index, body fat mass, body fat percentage, waist to hip ratio, visceral fat area, obesity degree, and blood triglyceride levels respectively (P 〈 0.05). Conclusion Patients with elevated body mass index, body fat mass, body fat percentage, waist to hip ratio, visceral fat area, obesity degree, and blood triglyceride levels, should be alert to the occurrence of nonalcoholic fatty liver disease in patients with type 2 diabetes mellitus. Body mass index, blood triglyeeride levels are independent risk factors for nonalcoholic fatty liver disease in patients with type 2 diabetes mellitus.
