摘要
目的探讨原发性高血压(高血压)患者左心房内径与血清谷氨酰转肽酶浓度的关系及临床意义。方法 2013年4月至2015年12月从鹤山市人民医院选取资料完整的原发性高血压患者200例,根据经胸超声心动图检查结果分为左心房增大组及左心房正常组。测量患者肝、肾功能,血脂,血压,血糖等指标并进行统计分析。结果在高血压患者当中,左心房增大组血清谷氨酰转肽酶浓度明显低于左心房正常组,差异有统计学意义[(21.56±11.95)U/L vs.(37.28±12.64)U/L,P=0.001]。Spearman相关分析表明血清谷氨酰转肽酶浓度与左心房内径呈负相关(r=-0.413,P=0.002)。多因素线性回归分析显示血清谷氨酰转肽酶浓度与左心房内径呈负相关(β=-0.394,95%CI:-0.131^-0.692;P=0.005)。结论高血压患者血清谷氨酰转肽酶浓度与左心房大小呈负相关关系,监测血清肝酶变化可能对早期发现心房增大有一定意义。
Objectives To analyze the relationship and clinical meaning of serum concentration of glutamyl transpep-tidase with left atrial diameter in patients with essential hypertension. Methods All the 200 newly diagnosed essential hypertension patients were from April 2013 to December 2015 in Heshan People ' s Hospital. They were divided into normal left atrial group and enlarged left atrial group based on the size of left atrial diameter detected by transthoracic echocardiography. Fasting blood glucose,serum lipid,routine laboratory tests,hepatic and renal function and blood pressure were measured by routine methods ,and used for analyzing between groups. Results Serum concentration of glutamyl transpeptidase was significantly lower in enlarged left atrial group than in normal left atrial group [(21.56 ± 11.95)U/L vs.(37.28±12.64)U/L,P=0.001]. Spearman correlation showed that serum concentration of glutamyl trans-peptidase negatively associated with left atrial diameter (r=-0.413,P=0.002). Multiple linear regression analysis showed that serum concentration of glutamyl transpeptidase also negatively correlated with left atrial diameter(β=-0.394, 95%CI:-0.131,-0.692;P=0.005). Conclusions Serum concentration of glutamyl transpeptidase negatively correlate with left atrial diameter in patients with essential hypertension ,and monitoring the changes of serum liver enzyme may has significant effect on the early detection of enlarged left atrial diameter.
