三叉神经-颈反射检测方法的建立及其对脑干梗死的诊断价值 被引量:1

摘要 目的 建立三叉神经-颈反射的肌电检测方法,探讨其在脑于梗死中的诊断价值。方法 受检测者取仰卧位,头部轻度抬高。刺激一侧眶下神经,于双侧胸锁乳突肌记录。结果 刺激对照者一侧的眶下神经,可于双侧胸锁乳突肌引出正/负波。脑干梗死组患者波形正常者7例(31.8%),波形消失者5例(22.7%),波幅降低者2例(9.1%),潜伏期延长者2例(9.l%),双侧反射的超常不对称者6例(27.3%)。结论 TCR可作为低位脑干病变的一种辅助检查手段。 Objective To study the physilogical behavior and the value of the trigemino-cervical reflex in the diagnosis of infarction of brain stem. Methods The subjects held the heads slightly raised when lying supine to make the sternocleidomastoid muscles tonically active. Electrical stimuli were applied to the infraorbital nerves and recorded from the sternocleidomastoid muscles. Results In all control subjects, stimulation of the infraorbital nerve on one side produced bilateral positive and negative waves. In the group of infarction of brain stem , 7 pa-tients showed normal(31. 8%), 5 patients showed nothing(22. 7%), 2 patients were reduced in amplitude(9. 1%),2 patients were delayed in latency(9. 1%) and 6 patients were above normally asymmetry in the response on two sides(27. 3%). Conclusion The trigemino-cervical reflex can be reliably measured. It may help in the discov-ering and dislocating lower brain stem lesion.
