Monitoring and Revision of Electrode Dislocation in Deep Brain Stimulation Surgery of Subthalamic Nucleus in Parkinson's Disease

摘要 目的探讨帕金森病(PD)丘脑底核(STN)脑深部电刺激(DBS)术中电极位置的监测和校准。方法对20例帕金森病患者进行双侧STN DBS治疗,植入40侧电极,通过术中微电极记录(MER)和X线成像进行电极位置的监测和校准。结果所有患者的术后程控效果良好,术后1年,患者的UPDRSⅢ总分在开机未服药状态下明显下降(P<0.05)。结论通过MER和X线成像可以监测到靶点的偏差和电极的移位并实时纠正,提高手术疗效,减少手术并发症。 Objective To explore the monitoring and revision of electrode in STN DBS surgery for Parkinson's disease (PD) patients. Methods A total of 20 cases of PD patients were input 40 electrodes in bilateral STN DBS surgery. Intraoperative microelectrode recording (MER) and X-ray imaging were used to monitor and revise the location of electrodes. Results All patients had good postoperative program control efficacy. 1 year after surgery, the total score of UPDRS 1I in the state of power-on and medication-offdecreased significantly (P〈0.05). Conclusions The application of MER and X-ray imaging can monitor the deviation of target and the shift of electrode, and correct the dislocation in real time, so as to improve surgical efficacy and reduce surgical complications.
