摘要
目的探讨阴式手术在剖宫产瘢痕部位妊娠患者中的临床效果及价值。方法取2014年1月~2016年12月医院妇科收治剖宫产瘢痕部位妊娠患者80例,根据手术治疗方法不同分为腹式手术组(n=35)、阴式手术组(n=37)和腔镜手术组(n=8)。腹式手术组采用传统开腹手术治疗,阴式手术组采用阴式手术治疗,腔镜手术组在腹腔镜下进行手术,比较临床疗效及术后并发症发生率情况。结果阴式手术组术中出血量、手术时间、hCG恢复正常时间及住院时间,均短(少)于腔镜手术组和腹式手术组(P<0.05);腔镜手术组术中出血量、手术时间、hCG恢复正常时间及住院时间、均短(少)于腹式手术组(P<0.05);阴式手术组与腹腔手术组并发症发生率比较差异无统计学意义(P>0.05),低于腹式手术组(P<0.05)。结论剖宫产瘢痕部位妊娠患者采用阴式手术、腹式手术及腔镜手术均能取得预期治疗目的,但是阴式手术更加快捷、微创、并发症发生率更低,值得推广应用。
Objective To investigate the clinical effect and value of vaginal surgery in cesarean scar pregnancy. Methods From January 2014 to December 2016, 80 cases of cesarean scar pregnancy in gynaecology department of our hospital were selected, and according to surgical treatment were divided into abdominal surgery group （n=35）, vaginal surgery group （n=37） and laparoscopic surgery group （n=8）.The patients of abdominal operation group were treated with traditional laparotomy, and the patients of vaginal operation group were treated by transvaginal surgery. The patients of laparoscopic surgery were treated by endoscopic surgery, and the clinical efficacy and postoperative complication rate were compared. Results The intraoperative bleeding volume, operation time, hCG recovery time and hospitalization time in vaginal surgery group were shorter （less） than those in laparoscopic surgery group and the laparotomy group （P 〈 0.05）. The intraoperative bleeding volume, operation time, hCG recovery time and hospitalization time in laparoscopic surgery group were shorter （less） than those in laparoscopic surgery group （P 〈 0.05）. The complication rate in vaginal surgery group and laparoscopic surgery group had no significant difference （P 〉 0.05）, and lower than that in abdominal surgery group （P 〈 0.05）. Conclusion Cesarean scar pregnancy patients with vaginal surgery, abdominal surgery and endoscopic surgery can achieve the desired therapeutic goal, but transvaginal operation is minimally invasive and has more efficient and less complication, it is worthy of popularization and application.
